By checking box below and submitting this form you are stating: I am aware that during dance lessons (or other related activities), that I/my child will be taking the floor at my/their own risk and agrees to hold Illusion Dance Academy (instructors and staff) harmless of any physical injury to the student. In the event of an emergency, every effort will be made to contact parents immediately. If unable to reach you, this form authorizes Illusion Dance Academy to call 911. This release will be effective for the entire period I am, or my children, are on the premises of Illusion Dance Academy or participant in off-facility locations. Also, the studio may take photos/videos of students for the purpose of promoting the studio, documenting progress and performances.
