Enrolment in Sallys School Of Performing Arts (SSPA) requires that you agree to the following:

- I have read, understood and agree to the conditions in the Class Information Pack on our website
- I give permission for SSPA to use photographic and/or videotaped images of my child. I understand that these images may be posted on social media sites, unless **Photo Opt-Out** is selected in the parent portal under More>Waivers & Policies
- I understand for my child to be enrolled at SSPA, a working email account is imperative to receive critical information distributed by SSPA
- I agree to respect the rights, dignity and worth of fellow participants, teachers, officials and spectators. I will treat everyone equally regardless of sex, disability, ethnic origin or religion
- I agree to the fee schedule in the Class Information Pack and understand the minimum enrolment period is one term regardless of participation
- I understand that SSPA reserves the right to suspend classes until Term fees are paid
- I understand the costume levy conditions as per the Class Information Pack and that this fee is non-refundable after the commencement of Term 3

I consent to my child receiving any medical attention deemed necessary by a medical practitioner and administer such first-aid as the teacher-in-charge judges to be reasonably necessary.

All parents please agree to the following:
- All parents give permission for SSPA to use photographic and/or videotaped images of my child unless **Photo Opt-Out** is selected in the parent portal.
- Parents to ensure they have read and understood the updated 2025 Class Information Pack available on our website.
- Parents are to ensure they have a working email account to receive critical information distributed by SSPA.
