Agreement for Participation
I have received information regarding studio policies, class fees and payment options, studio calendar, and student dress code and agree to adhere to all the content stated therein. I understand I can access these documents anytime online via the Parent Portal. Dancers and their family members who do not follow studio policies may be asked to leave the studio at the owners discretion.
The Dance Workshop is converting from a sole proprietorship to an incorporation August 2024. Our new legal name will be changing to The Dance Workshop Inc. When referring to the studio throughout this waiver we continnue to refer to the business as The Dance Workshop or TDW for the sake of simplicity.
I authorize The Dance Workshop to use my child's /my photo for promotional purposes without remuneration. Examples of use include but are not limited to: The Dance Workshop's Website, Facebook Page or Sponsored Ads, Instagram, Newspaper Ads (digital or hardcopy), Pre-Show Ads at the Circle Theatre or Imagine Cinemas. I understand that The Dance Workshop's year end recitals are filmed. Dancer's participating in recital will be filmed and the video recording will be distributed to all parents by digital download. The Dance Workshop may also use video clips from recital, class or studio events for promotional purposes. Dancer participation in recital is voluntary.
I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, hands on corrections from instructors, partner work/spotting, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. If registering for Acro I understand that by its nature Acro dance has a greater risk of injury than other dance styles.
I hereby release The Dance Workshop and The Dance Workshop Inc, its owner, staff, guest teachers, student assistants and volunteers from all liability in the event of any illness/accident/injury/death occurring while my child or I participate in or attend any class/studio event (ex. recital, parades, community performances) I understand that I should be aware of my physical limitations, as well as those of my child and agree not to exceed them. I certify that I am the parent/legal guardian of the child or children I am registering for and I have the right to waive these rights.
Despite our careful attention to disinfection, sanitizing, enhanced cleaning practices and social distancing practices there is still a chance you or your child could potentially be exposed to an illness on The Dance Workshop's property. Although The Dance Workshop has taken extra precautionary measures to reduce the possibility of transmission of communicable diseases, there is still a possibility that exposure may occur. Participation in classes, workshops, performances or activities hosted by The Dance Workshop is at your own risk.
I agree to abide by the studio policies and understand they are created for the safety and well being of all dancers and their families. Furthermore, I agree to The Dance Workshop's safety measures and guidelines to help stop the transmission of communicable diseases, including COVID-19.
I agree to pay for class fees and costume(s) following the terms set out in The Dance Workshop's payment plans. Late payments will be subject to a $10 late fee. Credit card payments are subject to a 3% processing fee. The Dance Workshop reserves the right to refuse service for non-payment of classes or costumes until the families account is brought up to date.
I hereby acknowledge that I have read and understand the statements above and agree to the terms for participation.
