I understand that my child is being enrolled into a program that uses physical activity. I have agreed that my child is in good physical condition and does not suffer from any condition that would prevent or limit participation in the program.

I understand that I am to assume full responsibility for all risks of physical injury from participation in our program.

I am aware that this is a release of liability and understand all risks to my child in connection with any instruction and further release Beyond The Boogie Dance Company LLC from liability for any injury or illness sustained by my child while he or she is enrolled in any program with Beyond The Boogie Dance Company LLC

I acknowledge and agree that it is my responsibility to acquire and maintain my own insurance coverage that provides coverage for myself and my child/children participating in Beyond The Boogie Dance Company LLC activities. I understand that Beyond The Boogie Dance Company LLC does not provide accident or health insurance for its participants or guardians of participants.

I the parent or legal guardian grant Beyond The Boogie Dance Company LLC my permission to use the photographs taken during Beyond The Boogie Dance Company LLC class or events as dance promotional pictures for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content.

Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

I agree to make non refundable tuition payments on or before the outlined date. If I fail to make payment, I understand I will incur a $25 late fee. I understand that I am responsible for payment of tuition should a circumstance arise that would prevent my child/children from participating in weekly classes.
