COVID-19 DISCLAIMER
Due to the COVID-19 pandemic, Miss Chloe’s School of Dance has explored different and reasonable ways to provide safe services to all students. We follow state and local agencies to implement safety in our studio. Though we will work hard to follow guidelines, neither the guidelines themselves nor even the guidance from the Centers for Disease Control and Prevention (“CDCâ€) guarantee an environment that is entirely free from COVID-19 related risks.
By attending or allowing your child to attend classes at our studio, you acknowledge and understand that your and/or your child’s attendance will require physical interaction and close contact with our staff members, other students and their families. As such, physical interaction with the public at large may pose some unavoidable risks to you, your child, and your family due to the COVID-19 pandemic. With that, you further acknowledge and agree to the following:
1. Waiver and Release: You hereby and forever discharge and hold harmless Miss Chloe’s School of Dance and its agents from any and all liability, claims and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from you or your child’s attendance and participation in any of our activities associated with or at our studio at any time. You understand that this release discharges the studio from any liability or claim that you may have against the studio with respect to COVID-19.
2. Assumption of the Risk: You further understand that your and / or your child’s participation and attendance may expose yourself or your child and others to unavoidable COVID-19 community spread. As such, you hereby expressly and specifically assume the risk of injury or other harm, and also expressly release the studio from all liability for injury, illness, or other issue resulting from or in any way related to your or your child’s participation.
