I authorise the staff of OnStage Dance Studios to consent to medical treatment for my child in the case of an emergency and when I cannot be reached. I am fully aware that any activity involving motion, height, or athletic activities creates the possibility of serious injury, and I further agree to hold OnStage Dance Studios and its staff and volunteers harmless for any injury or resulting expense(s). I release and discharge all rights and claims against OnStage Dance Studios. OnStage Dance Studios is fully insured and strives to provide the maximum in safety procedures and guidelines and will not assume responsibility for any accidents, injury, or illness that may occur.

OnStage Dance Studios will perform at studio watching weeks, demonstration days, photo days, annual concerts, as well as public performances and eisteddfods. During this time students may be photographed or videoed by both professionals (during the annual concert) as well as by families of the studio (during watching weeks, family day, class demonstration days as well as at eisteddfods and public performances). These photos and or videos may be displayed in the studio waiting area, studio newsletters, on our Facebook page, Instagram and may also be used in the annual concert slideshow.
Some selected photographs may be used for advertising purposes and publicity in the newspaper and or on our website. Although images and video footage may be used for advertising the students name and details will never accompany an image or the footage unless prior consent has been given.
All photos and videos will have the approval of the studio director Samantha Etcell and will be used in an appropriate and respectful demeanour.
I grant OnStage Dance Studios the right to reproduce, use, exhibit, display, broadcast, create and distribute images and video footage of my child who I am enrolling and understand it may be used in any media now known or later developed. I acknowledge that OnStage Dance Studios owns all rights to the images and video footage.

I agree to be responsible for tuition payments and fees until NOTICE OF WITHDRAWAL. I know that payment is due on or before the completion of week three of each term (specific dates are set for each individual term) and understand that there will be a $30.00 late fee for all payments received after this date. I am aware that all outstanding fees will incur a 10% late charge to the amount outstanding until the full amount has been paid for in full. I am also aware that there are NO REFUNDS or DEDUCTIONS for classes not attended however I am entitled to a makeup class within the same term as the class that was missed.
If withdrawal is necessary, I understand that advance notification will need to be given to OnStage Dance Studios and if the Dance Studio is not notified, my account will remain active, and I will still be responsible for payment of tuition regardless of the student’s absences. I also understand that any payment that remains overdue by two or more weeks will result in the Dancer/Student, not being permitted to participate in class and will not be able to attend any subsequent term until all fees due/ applicable are received and have been processed.
I understand that concert costume deposit invoices will be a deposit only and the balance will be due later in the year. The deposits for these costumes are $66 per student per mainstream classes and $110 for all students enrolled in our petite combo classes. Please understand that these deposits are non-refundable or transferrable. I understand that I need to OPT OUT of any concert season participation by the date provided in the newsletter. I understand I am responsible for the full cost of costumes if I haven't submitted an opt out email by the set date.
I also understand that OnStage Dance Studios will not be held responsible for any personal items that may be lost or damaged whilst at the studio.
I give consent for the student as listed in the enrolment process to attend classes at OnStage Dance Studios. I have read, understand and consent to the terms of enrolment and all policies set by OnStage Dance Studios.
