ALL STUDENTS AND THE PARENTS/LEGAL GUARDIAN ARE AWARE OF POSSIBLE PHYSICAL INJURY THAT MAY OCCUR DURING DANCE/TUMBLING CLASSES, PERFORMANCES, REHEARSALS AND ARE WILLING TO ASSUME THOSE RISKS. IT IS AGREED THAT NOSTALGIA DANCE CENTER, ITS OFFICERS, DIRECTORS AND ASSOCIATES ARE NOT RESPONSIBLE FOR PERSONAL INJURY OR PROPERTY LOSS.

Nostalgia Dance Center nor its employees will be held liable for any injuries or illness potentially sustained from participating in activities and classes at Nostalgia Dance Center.

All recreational classes run from August-June and require written notice 30 days in advance if you wish to drop a class.

I understand that tuition and fees are non-refundable and that all accounts with balances over 30 days will incur a late fee of $25.

Nostalgia Dance Center may use images and videos of my child for marketing purposes, included but not limited to: print, digital, social media, and other commercial campaigns, not including use by third party companies.
