ENROLLMENT & MEDICAL WAIVER

I verify that I am a legal/custodial guardian of the stated child listed under my account, or that I am at least 18 years of age.

I acknowledge that participation in any athletic activity is potentially dangerous, and there is an inherent risk of injury involved. I understand that dance, cheerleading, tumbling, and fitness related classes are all recreational sports, and as with any other recreational sport, there is a risk for injury. In the event that my child should receive an injury, I will not hold Eagle Dance Center nor the instructors/coaches responsible for such injuries.

In allowing my child to participate in classes at Eagle Dance Center, I hereby assume all the risks associated with the activities involved. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release Eagle Dance Center and it’s employees of any and all liability which may arise as a result of my child’s participation in activities at Eagle Dance Center.

In addition I agree to adhere to all content in the Parent/Student Handbook, including; Policies, Tuition, and Dress Code. I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.

I hereby acknowledge that I have read the statements above, and agree to participate accordingly.


PHOTOGRAPHY/VIDEOGRAPHY RELEASE

Parent Release Form for Photography and Videography
I give permission for Eagle Dance Center to use video footage and/or photographs of my child/ward listed on my account. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to:

- printed materials (example; brochures and newsletters)
- online and offline advertising and promotion
- videos and digital images such for use on Social Media and performance videography

By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Eagle Dance Center for a variety of purposes, and that these images may be used without further notification. I do understand that any identifying information will NOT be used in conjunction with any video or digital images.
