At Fascinatin Rhythm, I understand that there is no contract however I agree to give a 30 day notice in the event I need to unenroll my child. I understand that if a notice is not given I will be charged for the following month.

I also understand that costumes and recital fees are non refundable.

Our Backstage Staff are individuals that have been finger printed and background checked. We take our childrens' safety seriously.

Parents are not allowed in the backstage area during our annual dance recital.

I understand that there is a 3.05% plus .30 per transaction credit card processing fee added to my monthly tuition.

We understand this fee is annoying and we absolutely hate it. We keep our tuition prices low in order to help compensate for this fee that the credit card companies charge. We appreciate your understanding and look forward to providing you with amazing service.

On behalf of myself and my children, I understand that there are risks associated with, arising out of and inherent to the activities provided through Sarah Beth Studio of Dance II LLC dab Fascinatin Rhythm. They include, but are not limited to, illness injury and death. In recognition of this acknowledged risk, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Sarah Beth Studio of Dance II LLC dab Fascinatin Rhythm it’s officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Fascinatin Rhythm”.)

I hereby agree to release Sarah Beth Studio of Dance II LLC dab Fascinatin Rhythm harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks arising out of active participation in classes, performances and activities through Sarah Beth Studio of Dance II LLC dab Fascinatin Rhythm on behalf of myself, the legal guardian, and the participant/s.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the associated risks. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.

The participants for whom I am registering have my permission to participate in all Sarah Beth Studio of Dance II LLC dab Fascinatin Rhythm performances and activities. I warrant the information is complete and correct and that I do have the authority to act and speak in behalf of the participants I enroll in classes at Sarah Beth Studio of Dance II LLC dab Fascinatin Rhythm. I further release Sarah Beth Studio of Dance II LLC dab Fascinatin Rhythm of all liabilities associated with my childrens attendance at Sarah Beth Studio of Dance II LLC dab Fascinatin Rhythm.

During Our Annual Recital, proper Theatre Etiquette is required. For the safety of our Dancers the following Theatre Audience Etiquette will need to be practiced or theatre staff will ask the audience member to leave.

No talking, unless audience participation is requested by the performers. If something must be said, whisper it quickly.
No Flash Photography
No Video Taping (DVDs are included in your recital fee)
Use good posture
Remove hats
Phones Silenced
Avoid other sounds that can disturb the people around you and the performers.
In the event a small child is not being on their best behavior please escort them out of the theatre.
