I, the parent/guardian of the named participant, acknowledge that participation in dance is potentially dangerous and the is an inherent risk of injury involved. In allowing my child to participate in The Kaiser Center for the Performing Arts activities, I hereby assume all the risks associated with dance and the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release The Kaiser Center for the Performing Arts and it’s employees of any and all liability which may arise as a result of my child’s participation in activities at The Kaiser Center for the Performing Arts.
There are no refunds. In order to unenroll a dancer from class, they must unenroll by the 15th of the previous month. For example, in order for an account to not be charged via autopay on September 1st, the customer must turn in a drop notice by the 15th of August. A dancer can unenroll by filling out the drop form found in the Parent Student Handbook or you can contact our customer care team.
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing as well as strict cleaning protocols.The Kaiser Center for the Performing Arts (KCPA) has put in place many preventative measures to reduce the spread of COVID-19; however, KCPA cannot guarantee that you or your child(ren) will not become infected with COVID-19 by entering our facility.By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending KCPA classes and events and that such exposure or infection may result in personal illness. I understand that the risk of becoming exposed to or infected by COVID-19 at KCPA may result from the actions, omissions, or negligence of myself and others, including, but not limited to, KCPA employees, volunteers, and program participants and their families.I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at KCPA or participation in KCPA programming (“Claimsâ€). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless KCPA, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of KCPA, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any KCPA program.
I, the undersigned, give permission for The Kaiser Center for the Performing Arts to use video footage and / or photographs of my child/ward. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to: - printed materials (eg - brochures and newsletters) - online and offline advertising and promotion - videos and digital images such for use on Social Media. By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by The Kaiser Center for the Performing Arts for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.
