I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that Rysing Star Dance Academy LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Rysing Star Dance Academy LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other students and their families.
I voluntarily seek services provided by Rysing Star Dance Academy LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my classes.
I attest that when entering the studio:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

--->>> IF I HAVE BEEN EXPOSED- I will quarantine for 5 days to monitor symptoms before returning to classes regardless of vaccine status, my 90 day window, or if I am symptomatic.

I hereby release and agree to hold Rysing Star Dance Academy LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from Rysing Star Dance Academy LLC. I understand that this release discharges Rysing Star Dance Academy LLC from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Rysing Star Dance Academy LLC. This liability waiver and release extends to the studio together with all owners, partners, and employees.



The undersigned acknowledges and understands that with any physical activity there is a risk of injury to the participants. The undersigned understands that in consideration of being permitted to participate in the classes, programs, training and exhibitions involving various forms of dance and movement, does hereby release Rysing Star Dance Academy (RSDA), the owners, instructors, staff and volunteers in charge for all actions, demands or claims that may result because of injury.
Rysing Star Dance Academy has taken certain steps to implement recommended guidance and protocols issued by the Public Health Agencies for slowing the transmission of COVID-19, including, without limitations, the access/use restrictions set forth below. The undersigned understands and agrees that RSDA may revise its procedures any time based upon updated guidance and protocols issued by the Public Health Agencies and further agrees to comply with the revised procedures while participating in the programs and services at RSDA.
In further consideration of being permitted to enter or use the facilities or equipment, or participate in any on-site or off-site program affiliated with RYSING STAR DANCE ACADEMY (RSDA) the undersigned hereby agrees to the following:
The undersigned, and on behalf of such participating children, hereby releases, waives, discharges and covenants not to sue RYSING STAR DANCE ACADEMY, LLC (RSDA), the owners, instructors, staff and volunteers from all liability for any loss or damage, and any claim or demands on account of any property damage or any injury to, or an illness or the death of, the undersigned or such participating children (or any person who may contract COVID-19, directly or indirectly, from the undersigned or participating children) whether caused by negligence, active or passive, of RYSING STAR DANCE ACADEMY, LLC.
The undersigned hereby agrees to indemnify and save and hold harmless RSDA, the owners, instructors, staff and volunteers from any loss, liability, damages or costs they may incur whether caused by negligence, active or passive, while the undersigned or any participating children participating in any programs affiliated with RSDA. The undersigned understands that RSDA is not required to provide insurance to cover the participating children in the event they suffer illness, injury, death, property loss, theft or damage of any sort while participating in any program affiliated with RSDA. THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR, AND RISK OF ILLNESS, BODILY INJURY, DEATH OR PROPERTY DAMAGE to the undersigned or the participating children while using the premises of RSDA and/or participating in any programs affiliated with RSDA.
