By creating a Parent Portal account and enrolling my child in Dance Unlimited of Destrehan, I am agreeing to all policies and procedures set forth by Dance Unlimited of Destrehan, LLC and its staff. I also understand that before my child participates in any dance or tumbling class, I am required to accept the below liability waiver and respond to the below photography and social media release.

I understand that there are inherent risks of physical injury arising from dancing and/or tumbling. These risks include, but are not limited to slips and falls, sprains, strains, dislocations, soft tissue injuries, and muscular and/or skeletal injuries. I also understand that Dance Unlimited of Destrehan LLC does not employ any doctors, nurses, or other medical staff to examine, diagnose, or otherwise provide treatment or preventative medical care for any potential injuries that may occur from dancing and/or tumbling. I agree to accept these inherent risks associated with dancing and/or tumbling and that Dance Unlimited of Destrehan LLC is under no duty to provide any medical treatment for any injuries your child may potentially sustain. By agreeing to this document, I further agree to waive the right to seek damages against Dance Unlimited of Destrehan LLC, its officers, employees, agents, independent contractors, and insurers arising from any injuries your child sustains that are caused by the inherent risks of dancing and/or tumbling.

I understand and acknowledge that dance and tumbling education often times involves hands-on instructions, which includes the potential for instructors touching a dancer's arms, legs, feet, hips, head, and back to place them in the correct position. I acknowledge that such hands-on instruction is standard practice in the industry and by agreeing below, I give my consent to such instruction. I shall be responsible for advising an instructor beforehand if there is any form of touch unacceptable to me so that any reasonable accommodations or arrangements can be made accordingly.

Throughout the year photographs will be taken during DU classes and events. By accepting, you agree for your child to appear in these photographs which may be used in advertising to promote the business as well as on various forms of social media.
