North Star Academy of Dance requires a 15 day notice to cancel enrollment for all students. This notification allows North Star to make the necessary adjustments to the classes caused by the cancellation. Adjustments may apply to closing a class, combining a class, and modifying recital dances.

If the notification to cancel enrollment is less than 15 days prior to the last day of the month , the account will be charged the normal tuition amount for the upcoming month. If notification to cancel enrollment has been sent after tuition has already been paid (regardless if you are a new student who has not attended the 1st class yet or existing students), no refunds will be allowed. In this situation since the month has already been paid for, the student would be allowed to move to a different class if there is a scheduling conflict due to the request to withdraw from enrollment. By accepting this waiver, the parent acknowledges that refunds will not be allowed.

Adhering to our cancellation policy allows for proper planning to be done so that the impact to the class, student and teachers can be managed efficiently and hopefully without disruption.

ALL Notifications to Cancel Enrollment must be received by email and sent to the Studio Owner at admin@northstaracademyofdance.com.

The Annual Registration Fee is $35 for individuals & $50 for a Family (no limited on number of children). Unless it is otherwise noted, all classes will participate in the Annual Recital on 6/9/23 (dress rehearsal) & 6/10/23 (Show date). The Studio Year begins in August each year. Non-Recital classes include: Technique, Leaps & Turns, Flexibility & PrePointe. Class descriptions in the class schedule will identify the recital classes. Please contact the studio if you need to move to a non-Recital class if a student is unable to participate in the Recital.

The Recital/Performance Fee is $75 and due by 10/15 along with the Recital costume fee(s) or upon enrollment if after 10/15/22. This fee covers participation in both Holiday and Recital performances, digital download of the Recital performance only and 2 complimentary tickets to the Annual Recital. These are NON-REFUNDABLE FEES! Recital date is 6/10/23 with dress rehearsal on 6/9/23.

Waiver and Release of Liability I

I understand and agree that in participating in any dance class, workshop, rehearsal, event or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of North Star Academy of Dance classes, rehearsals, performances, or activities. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. I certify that there are no health-related reasons or problems which preclude my child’s participation in this activity or event.

I HEREBY WAIVE, RELEASE, AND DISCHARGE North Star Academy of Dance LLC and Karen Robinson, Owner and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, intensives, workshops, private instruction, choreography or any activity.

I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: Owner: Karen Robinson, North Star Academy of Dance LLC and/or their directors, officers, managers, employees, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers;

I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this waiver, release and registration form from any and all liabilities or claims made as a result of participation in activities or events, whether caused by the negligence of release or otherwise. The waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT, ACCEPT ALL TERMS OF MY OWN FREE WILL by indicating acknowledgment on this form.

Permission is granted to North Star Academy of Dance LLC to use photographs of students for publicity purposes.

Upon registration, you agree to be bound by the above statement
