I represent and warrant that the student is in good physical health and does not suffer from any medical condition(s) that would limit participation in the classes, workshops and events offered by Oregon Conservatory of Performing Arts (OCPA), either onsite, virtually or at an offsite location. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any of the dance classes, workshops or events offered by OCPA. I understand the risks associated with being in a public space and participating in the activities offered by OCPA. I acknowledge that participation in classes exposes me to possible risks of personal injury. I am fully aware of these risks and hereby release OCPA and/or any of their instructors from any and all liability, negligence, or other claims arising from, or in any way connected with my participation in their classes and any other events and workshops offered by them. I have read the above release and waiver of liability and fully understand its content. I am legally competent to sign and voluntarily agree to the terms and conditions stated above. I agree that OCPA and Camelot Theatre are in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes taken with OCPA may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death.
I understand that OCPA operates in public facilities in which airborne viruses may be present. I agree that I will not hold OCPA reliable for any illness which may be contracted by visiting a public facility and being in proximity with others, and for any allergic reactions to airborne remnants of cleaner within their properties. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against OCPA or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
By signing below, I agree to each statement above and release OCPA from any and all liability for the unintentional exposure or harm due to COVID-19.
Our Magic of Theatre Summer Camp is being held at South Medford High School, and we will continue to adhere to Medford School District Guidelines. The following covid guidelines will be implemented until further notice:
Maintain contact logs in order to identify and isolate COVID-19 cases and exposures (state requirement).
We will continue to work with JCPH to identify COVID cases as well as those considered exposed to COVID, and will be required to quarantine affected unvaccinated students and/or staff.
Set up spaces for three-feet physical distancing to the maximum extent possible, while not limiting activities due to distancing.
Make face coverings optional for campers and staff.
Continue other safety measures, which include:
Increased fresh air intake and upgrades to MERV-13 filters, education and training, and dedicated school COVID safety teams.
