The 8th Count Dance Center requests that each student enrolled consult with a physician in respect to any past or present illness or injury that may affect his/her participation in dance exercises and all other activities at The 8th Count Dance Center.

As with all physical activity there is a risk of injury with dance and tumbling. Even though The 8th Count Dance Center will take every precaution to assure a safe environment, physical activities such as those taught or participated in at The 8th Count Dance Center have risk of injury. I agree to be solely responsible for any and all costs, damages and expenses incurred by me and/or my child as a result of any injury sustained from any class participation at The 8th Count Dance Center. I further agree not to hold The 8th Count Dance Center, its staff and/or instructors and guest instructors responsible in any way for such injury.

If a parent or guardian is not present, I authorize The 8th Count Dance Center staff, in the event of injury or illness, to administer emergency care and to arrange for any emergency medical transportation to the nearest health care facility. I understand every effort will be made by the staff to contact a parent, guardian or emergency contact before any involved treatment. I give permission to a qualified physician and/or other medical personnel to administer medical care while myself/daughter/son is participating in activities at or for The 8th Count Dance Center. I also agree that my insurance carrier or myself will bear the financial responsibility for any medical treatment given under the above guidelines.

By signing this waiver I agree to hold The 8th Count Dance Center, its staff and/or all instructors harmless for any injury, which might occur on the premises, during any instructional session related to The 8th Count Dance Center (i.e. workshops, performances, competition, recital etc.). The parent/adult signing is assuming any and all responsibility for the student, including financial obligations. As the parent, by signing this form, you understand all policies and procedures of The 8th Count Dance Center. If my class fees, costume fees and all other fees are not paid by the year end recital, I understand that my child will not be allowed to participate in the recital until the fees are up to date.

COVID-19 ADDENDUM 2020-2021
ASSUMPTION OF THE RISK AND WAIVER OF LIMITED LIABILITY RELATING TO CORONAVIRUS/COVID-19

The 8th Count Dance Center has put in place preventative measures to reduce the spread of COVID-19, however, The 8th Count cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending classes could increase your risk and your child(ren)'s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed or infected by COVID-19 by attending The 8th Count Dance Center. I agree to comply with all The 8th Count policies and rules, including but not limited to all The 8th Count guidelines, signage and instructions. I understand that the risks of becoming exposed to or infected by COVID-19 at The 8th Count may result from the actions, omissions or negligence of myself and others, including but not limited to The 8th Count employees, volunteers and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility of any injury to my child(ren) or myself. I agree to hold harmless The 8th Count Dance Center from and against any and all costs, expenses, damages, claims, lawsuits, judgments losses and/or liabilities (including attorney fees) arising either directly or indirectly from or related to any and all claims made due to bodily injury, death, loss of use, monetary loss, or any other injury from or related to the use of The 8th Count Dance Center facilities, whether caused by the negligence of The 8th Count Dance Center or otherwise specifically related to COVID-19.

I am sufficiently informed about the risks involved in attending The 8th Count Dance Center to decide whether to sign this document; no oral representations, statements, or inducements apart from the foregoing agreement have been made. This waiver remains in effect until the state of Ohio lifts all COVID-19 related mandates.

I am enrolling my dancer for The 8th Count Dance Center for the 2020-2021 Dance Season. I understand that tuition is based on a yearly tuition of approximately 36-40 weeks broken down into 9 monthly installments. There are also costume deposits due throughout the year. I understand that if my child withdraws from the program, that registration, costume deposits and monthly tuition is 100% nonrefundable. I also understand that I am responsible for all past due tuition.

By signing this form I have received the dress code and the policies packet set by The 8th Count Dance Center. I also give my consent to use my child's photograph for advertising purposes only through publication, social media, or any other advertising purpose. I understand that it is my responsibility to update any personal information with the studio.
