Release and Authorization
I grant permission to the staff of the dance school to take first aid or emergency measures as judged necessary for the care and protection of my child while under the supervision of the school. In case of medical emergency, I understand that my child will be transported to an appropriate medical facility by the local emergency unit for treatment if the emergency unit deems it necessary. I understand that in some medical situations the staff will need to contact the emergency resource before the child's parent, physician, and or other person acting on the parent's behalf. I also understand and agree that the child's parents or legal guardians shall be responsible for any expenses incurred. As the parent/legal guardian I also agree to hold harmless from any and all liability the school, its officers, employees both in their professional capacity and personally for all injury or illness resulting from or in any way connected with his/her participation in the classes, activities or special events at the school. I understand that it is the school's policy that while under the supervision of the school no child is allowed to leave the building without a parent/legal guardian or the written permission of a parent/legal guardian and that the parent/legal guardian assumes full responsibility for the actions and behavior of the child. Parents/legal guardians give their permission to the school to use photos and or video of their child without remuneration in connection with school publications, advertising, tv and news coverage. Registration is for the full September to May dance season and all payments are nonrefundable.

COVID-19 Studio Policy and Procedures
(This does not represent our procedures in their entirety, please see our policies and
procedures from registration)
1. I agree that I will not enter the studio if I have been sick or if a family member or I have
had a fever in the past week.
2. I agree to have my temperature checked upon entering studio, if temp exceeds 100.4 I
will not be allowed to enter building.
3. I agree all shoes will be removed at the door and returned when I leave (no street shoes
in dance room).
4. I agree to be respectful to those who choose to wear a mask and to those who choose
not to.
5. I agree to wash or sanitize my hands before entering the building and after using the
restroom, before eating or handing to children meals/snacks, and upon leaving the
studio.
6. I agree to keep my hands and belongings to myself.
7. I agree if I begin to feel ill and or coughing, I will remove myself from the studio.
8. I agree to practice social distancing and to the best of my ability remain 6ft away from
other people.
9. I understand that the lobby will remain closed, no observers during this time.
10. I understand students entering the studio will need to enter from the front of the studio
and exit out the side doors. Students will be picked up from the side of the studio.
11. I have explained to my child that there is NO sharing of food or drinks.
12. I understand that the classes will be slightly shorter than previously held in order to
allow us time to sanitize between classes.
13. I understand that we will be continuing to process payments through online, phone or
lockbox.
14. I understand that these rules can change from time to time.

Waiver and Release of Liability
PLEASE READ BEFORE SIGNING
In consideration of being allowed to participate in any way in Attitudes Dance Studio’s program, related
events and activities including being a spectator in studio or events, the undersigned acknowledges,
appreciates and agrees that:
1. The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from
the activities involved in this program are significant, and while particular rules, equipment, and personal
discipline may reduce these risks, the risks of serious injury and illness do exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, even if arising from the
negligence of the releases or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If,
however, I observe any unusual significant hazard during my presence or participation, I will remove
myself from participation and bring such to the attention of the nearest office staff immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE
AND HOLD HARMLESS Attitudes Dance Studio their employees, office staff, volunteers, other participants,
sponsoring agencies, and if applicable, owners and lessors of premises used to conduct the event
(“RELEASES”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to
person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE REALEASEES OR OTHERWISE, to
the fullest extent permitted by law including all liabilities, claims, actions, damages, costs, or expenses or
any kind arising out of or relating thereto, and
5. I understand and agree that this release includes and claims based on the actions, omissions, or
negligence of Attitudes Dance Studio, its employees, agents, and representatives, whether an infection
(such as COVID-19 disease or similar infection) occurs before, during or after participation in any studio
program, and
6. I can attest (and will continually monitor-i.e.. Through temperature taking, visually monitoring etc.) that
my child, nor anyone in the household (for a minimum of a week) has/have any physical health conditions
that present the risk of transmitting any virus/or disease to other participants in these activities.
7. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND
ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT
FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
I have also read the studio regulations and the attached sheet and agree to abide by all the rules and
to explain the rules to my child(ren).
This waiver does not represent our full waiver of liability (see registration card) and in no way should
nullify any previous waiver of liability signed through Attitudes Dance Studio.

FOR PARTICIPANTS OF MINORITY AGE
(UNDER AGE 18 AT THE TIME OF REGISTARTION)
This Is to certify that I, as parent/guardian with legal responsibility for this participant, have
read and explained the provisions in this waiver/release including the studio policies and
procedures to my child/ward including the risks of the activity and his/her responsibilities for
adhering to the rules and regulations. Furthermore, my child/ward understands and accepts
these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree
to his/her release provided above for all the Releases and myself, my spouse, and child/ward do
release and agree to indemnify and hold harmless the Releases from any and all liabilities
incident to my minor child’s/ward’s involvement or participation in these activities as provided
above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.
