Must be over 18 to register for an account

I authorize the employees of Rising Stars to seek medical attention for my child as identified above in the event of my absence. I also authorize attending medical personnel to render such immediate emergency medical treatment as may be required. I further hereby agree that Rising Stars and its employees shall not be held liable for injury or illness to my self or my child due to any accident, communicable virus or any other cause outside of its control which may occur during participation in any activities held at, sponsored by or in any way affiliated with any of the entities of Rising Stars. I also understand that non-agreement makes my child ineligible to participate.
