You must be over 18 to register for an account, and will agree to our polices and waiver with release from liability for dance instruction prior to registering with Elite Dance Centre.

- I understand that all pages of this registration form packet must be initialed at the bottom and returned meaning I fully understand the contents each page displays.
- I understand that Elite Dance Centre, LLC reserves the right to refuse instruction to anyone not abiding by Elite Dance Centre, LLC policies.
- I understand that Elite Dance Centre, LLC is not responsible for lost items, stolen items or unclaimed items.
- I understand that all adults and dancers must act appropriately at all times or an immediate dismissal may occur. POSITIVITY ONLY!
- I understand that all fees must be paid in full according to my payment schedule I have chosen and that all tuition fees must be paid on time, even if I or my child were dismissed during the dance season at any time due to behavior.
- I understand that all choreography is property of the studio, therefore any performances or functions outside of the studio must have permission from the studio owners.
- I understand that regular attendance is vital for student progress and group choreography. If it becomes a problem your instructor may be forced to dismiss you from the concert routine / dance team.
- I agree to pay any and all fees associated with the cost of collection, including reasonable attorney’s fees, on any outstanding balances owed to Elite Dance Centre, LLC. I understand this is also in the event my child is dismissed due to my or my child’s non-compliance with Elite Dance Centre, LLC policies.
- I understand that all fees associated with Elite Dance Centre, LLC are non-refundable.

By agreeing below I have chosen to have my child / children, participate in dance instruction given by Elite Dance Centre, LLC. I acknowledge that I understand the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that Elite Dance Centre, LLC is not in any way responsible for making such a determination.

In consideration of my child / childrens' enrollment in any dance instruction program, I understand and agree on behalf of myself and my child, to release, hold harmless, and discharge Elite Dance Centre, LLC from all claims, costs, liabilities, expenses or judgments, including attorney’s fees and court costs for any occurrences in connection with any dance instruction. I assume all risks to my child in connection with any instruction and further release Elite Dance Centre, LLC and its owners and employees from liability for any injury sustained by my child / children while he or she is enrolled in any dance instruction program, including all risks reasonably connected with such activity whether foreseen or unforeseen.

I understand that Elite Dance Centre, LLC is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the dance studio and that Elite Dance Centre, LLC will only be supervising my child / children when he or she is participating in scheduled dance activities, programs or instruction.

I understand that Elite Dance Centre, LLC is not responsible for personal property that is lost, damaged or stolen while I or my child / children are at or on Elite Dance Centre, LLC property.

I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for myself and my child / children participating in Elite Dance Centre, LLC activities and that Elite Dance Centre, LLC does not provide accident or health insurance for those participating in its instruction, activities or programs.

I authorize and agree that Elite Dance Centre, LLC may take and use photographs, videos or likenesses of myself or my child / children as needed for its record- keeping, advertising and/or public relations projects and that I have no rights to the same and will not be compensated for the same.

I hereby release Elite Dance Centre, LLC, its agents and employees from all liability for personal injury, illness or property damage occurring on or off the premise leased by Elite Dance Centre, LLC, whether or not caused by negligence of Elite Dance Centre, LLC, its agents or employees. I certify that I am in good health and capable of participation in all activities and classes. In an emergency, I authorize Elite Dance Centre, LLC to take such temporary measures as Elite Dance Centre, LLC, deems appropriate. I agree to pay my account in full when due, even if my child / children are dismissed at any point throughout the dance season due to any reason deemed inappropriate by Elite Dance Centre, LLC. I also agree to pay any and all fees associated with the collection of any outstanding balances on my account- even if classes are dropped.

Information on Elite Dance Centre, LLC will only be released to the parents or guardians that register the child.
My agreement is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to agree to this affirmation.

I HAVE FULLY INFORMED MYSELF AS TO THE CONTENTS OF THIS RELEASE AND HAVE READ THE SAME PRIOR TO AGREEING.
