Liability Waiver &
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19
for Summer Classes/Intensives/Drop-In Classes/Master Classes/Private Lessons

-I acknowledge that I understand the nature of the activities my child(ren) will be participating in and that my child(ren) is in a healthy condition and capable of participating in the related activities. I understand that Valentina’s School of Ballet is not in any way responsible for making that determination.
-In consideration of my child(ren)’s enrollment in any dance instruction or program, I understand and agree on behalf of myself and my child(ren), to release, hold harmless, and discharge Valentina’s School of Ballet, its owners, offices, staff, employees and independent contractors from all claims, cost, liabilities, expenses or judgments, including attorneys, fees and court costs for any occurrence in connection with any dance instruction or programs at Valentina’s School of Ballet.
-I assume all risks to my child(ren) in connection with any instruction or participation in programs and further release Valentina’s School of Ballet, its owners, offices, staff, employees, or independent contractors from any liability for any injury sustained by my child(ren) while he or she is enrolled in any dance instruction or program, including all risks reasonably connected with such activity whether caused by negligence or any other cause foreseen or unforeseen.
-In the event of any emergency, I authorize VSB/staff to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed reasonable and necessary for my minor child’s immediate care when parent, guardian, or emergency contact cannot be reached to authorize the treating physician to provide such emergency medical services.
- I understand that I am fully responsible for payment of any and all medical services rendered and that the absence of medical insurance does not make Valentina’s School of Ballet responsible for paying medical expenses.
- I understand that dance instruction involves kinetic corrections to the body that may involve physically touching the student as part of regular class work and rehearsals for the purpose of correction of dance technique, body alignment, turn-out, and placement. I give my permission for this instructional contact to be made as part of my child(ren)’s training and agree to hold harmless and release Valentina’s School of Ballet from any liability regarding such.
-I understand that Valentina’s School of Ballet is not responsible for my child(ren) or other children under my supervision who are left unsupervised in the common areas and areas surrounding the dance studio.
- I understand that Valentina’s School of Ballet is not responsible for personal property that is lost, damaged, or stolen while I or my child(ren) are on Valentina’s School of Ballet property.
-I authorize and agree that Valentina’s School of Ballet may take and use photographs or videos of my child(ren) as needed for its record-keeping, advertising, social media and/or public relations projects and that I hereby release Valentina’s School of Ballet, its owners, offices, staff, employees and independent contractors from all claims and demands whatsoever arising out of in connection with the foregoing and waive any and all rights to payments for use of these photographs and/or videos.
-The novel coronavirus, COVID-19, was declared a pandemic by the World Health Organization in March 2020. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. Valentina’s School of Ballet cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, having your child(ren) attend dance classes (Summer/Intensives/Master or private/semi-private lessons in the studio could increase your risk and your child(ren)’s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending classes/Summer Programs and/or private/semi-private lessons at Valentina’s School of Ballet and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the studio may result from the actions, omissions, or negligence of myself and others, including, but not limited to, VSB employees, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Valentina’s School of Ballet or participation in any summer program. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Valentina’s School of Ballet, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Valentina’s School of Ballet, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Valentina’s School of Ballet class/program and/or private/semi-private lessons.
-By signing this waiver or completing the online/paper registration form is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein an agreement as to all terms and conditions contained above.

-I understand that Costumes are the property of Valentina’s School of Ballet and must be returned right after recital on the assigned date. The costumes being returned need to be clean, in good condition and spotless. All accessories (tiaras, gloves, hair pieces, jewelry) must be returned with costumes in a ziplock plastic bag.
-I agree to return the costumes on the original hangers and if the costume or any accessory is lost a replacement fee will be assessed (value of the item, $10 - $600).

As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Valentina’s School of Ballet has put in place preventative measures to reduce the spread of COVID-19; however, Valentina’s School of Ballet cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, having your child(ren) attend dance classes/Summer Programs or private/semi-private lessons in the studio could increase your risk and your child(ren)’s risk of contracting COVID-19.
