In consideration of the benefits derived from Diversity Dance Studio, I (we) do hereby agree to hold harmless Diversity Dance Studio, its Directors, employees, agents, independent contractors, successors and assigns, from any claims for personal injuries to myself, my (our) boys or girls while participating in said dance or any other studio activities. PHOTOGRAPHIC/MEDIA CONSENT: I hereby consent to the use of photographs or video footage for use on the Diversity Dance Studio website, social media, in newsletters and publications as well as for distribution to members. I give this consent voluntarily and understand that I may withdraw my consent at any time upon written notice.
