By agreeing to this policy you acknowledge the receipt of your 2023-2024 Dancer Handbook that can be located in the "Shared Files" section of your parent portal. Please read through the handbook prior to the start of the season so you are aware of studio polices, procedures and important dates.

Personally Identifiable Information
Personally Identifiable Information collected on the enrollment form will only be used for the day to day functions of Miami Valley Dance Center. Including but not limited to posting first and last names on the internet and social media for photos. Printing first and last names for recognition in the Miami Valley Dance Center newsletter, DANCE!.

Contact Information
Contact Information collected on the enrollment form will only be used for the day to day functions of Miami Valley Dance Center. Including but not limited to sending email messages or making phone calls for the purpose of delivering information related to your participation at Miami Valley Dance Center. Emergency contact information will only be used in the case of an emergency. Miami Valley Dance center will never share/sell your information to third parties.

Credit Card Information
Miami Valley Dance Center implements an autopay/monthly subscription style payment collection. All credit card information is stored with STRIPE (Card Processor). Once information is placed with STRIPE, it is truncated to the last four digits and unreadable by ANYONE at Miami Valley Dance Center. An ID is assigned, and charges are made against the ID number. Miami Valley Dance Center DOES NOT store credit card information (as defined by PCI Security Standards).

Miami Valley Dance Center reserves the right to refuse enrollment and to terminate services for any reason
Enrollment and placement in class is subject to instructor approval
Miami Valley Dance Center maintains a website, Instagram and Facebook page and may post student's pictures
Miami Valley Dance Center reserves the right to cancel or reschedule a class due to insufficient enrollment
Miami Valley Dance Center and it's instructors will not be held liable for any injuries, illness, or property damage which may occur during the course of instruction
No refunds will be given
All dancer accounts must have a valid credit card on file. Monthly Payments will be drafted on the first of the month
A 10% late fee will be assessed on any outstanding balance after the 5th of the month
There is a $35.00 service charge on returned checks
There is a 3.05% + .80/transaction convenience fee on credit card payments, .8% on ACH payments
Services will be terminated for any account that is 60 days past due
Accounts must be paid in full by June 1, 2024 for participation in the year end Dance Extravaganza in June
I will download the MVDC dancer handbook and submit the parent/dancer agreement by October 31, 2023
