You must be 18 or over to register an account.

As a parent/guardian, and as a dancer in membership with the Revolve Dance Alliance Competitive Performance Company, we have read through the Competition Requirements, Terms and Policies, an understand the above Rules and Requirements to participate with Revolve Dance Alliance. Failure to follow the Requirements and Terms will result in dismissal from the team. I understand the time commitment and financial commitment associated with registering to participate in the Company. I understand that if I do not fulfill this contract through, I will be responsible for 2 months of tuition, after I stop coming to class. As the dancer, I understand that having a positive attitude is of the highest priority in class and crucial for a successful team. As the dancer I understand that gossip, purposefully excluding other dancers, and any type of bullying will not be tolerated and will result in immediate dismissal from the team.

If I cannot be reached in the event of an accident or emergency while the above is under the care and supervision of Revolve Dance Alliance, I hereby authorize and give permission to their staff/assigns to undertake and employ emergency first aid, emergency transportation, obtain emergency medical treatment, and act in my stead to follow such procedures as necessary to admit and treat any emergency condition, at any hospital, if it is deemed necessary. I attest that I have current and valid medical insurance and will be financially responsible for any and all ER medical expenses. Revolve Dance Alliance has my full permission to act as temporary guardian in an emergency situation. By signing this, I agree to hold harmless Revolve Dance Alliance and any hospital from any liability for commencing emergency medical treatment with any more consent then hereby given for and in consideration of my/our voluntary participation in the dance or related programs of Revolve Dance Alliance recognizing and attesting that all due legal warnings have been given, including the possibility of temporary or permanent injury, broken bones, etc. Having determined that appropriate precautions are and will be taken in connection therewith. I/WE HEREBY ASSUME ALL RISKS, WAIVE AND FOREVER RELEASE ANY AND ALL RIGHTS AND CLAIMS FOR PERSONAL INJURIES, MENTAL AND EMOTIONAL SUFFERING, PROPERTY DAMAGES, PUNITIVE DAMAGES AND LOSS OF SERVICE WHICH I/WE MAY HAVE, NOW OR IN THE FUTURE AGAINST REVOLVE DANCE ALLIANCE, ITS OFFICERS, ASSIGNS, AGENTS, EMPLOYEES, LANDLORDS, AND SUCCESSORS. THIS WAIVER SHALL BE BINDING ON MY FAMILY, HEIRS, EXECUTORS, ADMINISTRATORS AND ASSIGNS.

Please be reminded this form releases Revolve Dance Alliance and all staff members and officials from any claims of damages or injury suffered by the registered student (s) in connection with or by their association with Revolve Dance Alliance. I give my permission to Revolve Dance Alliance to use photos of my dancer online (for the website, Facebook, Instagram, etc.) or in an advertisement.
I understand that Tuition is due on the 1st of the month. A $15 late fee will be assessed for all payments made after the 7th of the month. No refunds are given for any reason. Tuition is not lowered if classes are unattended. If tuition is more than 45 days late the student will not be able to participate in classes. All balances on your account must be paid in full before receiving any costumes or performing. If an account goes 90 days past due, it is sent to collections.

RELEASE, WAIVER OF LIABILITY, ASSUMPTION OF RISKS and INDEMNITY/HOLD
HARMLESS AGREEMENT for use of REVOLVE DANCE CO. LLC

1. Defined Terms under this Agreement:
"Owner" - Revolve Dance Co. LLC
"Users" - shall mean each signatory to this document and each signatory's Minors, heirs,
successors, representatives and assigns.
"Minors" - shall mean the following minors who are the children, foster children, wards or
otherwise legal responsibility of the Parent/Guardian Users,

"Released Parties" - shall mean the Owner, Revolve Dance Co. LLC
"Property" - shall mean the Owner's building and outside parking area

2. The undersigned Users do hereby execute this Release, Waiver of Liability, Assumption of
Risks and Indemnity/Hold Harmless Agreement (the "Agreement") for themselves and their
heirs and any of their Minors, guests and invitees and acknowledges the inherent risks involved
in the use of the Owners' Property, which risks include, but are not limited to bodily injury.

3. Users acknowledge and agree that a Parent/Guardian shall be present on the Property to
accompany and supervise their minor child and/or children. The Parent/Guardian Users
assume all responsibility for the supervision, safety and welfare of their minor child or children
while upon the property.

4. In exchange for being permitted to use the property the undersigned Users release and
agree not to make or bring any claim of any kind against the Released Parties (Owners) for: (1)
any injury to Users, Minors or their guests and invitees, (2) any damage to personal property
whether or not caused by the negligence of the Released Parties or not; or (3) any other cause
arising out of or related to visit of Parent/Guardian, Users, Minors or their quests and invitees.

5. This waiver covers users and owners for all visits to the property from September 2021
forward.

6. Users give permission for use of images of Users or likeness on social media (photos, videos, posts, reels, stories, TikToks, promotions, advertisements, etc.) pages of the Owners (@revolvedancealliance).
