Your immune system is your best defense against any air-borne illness. While we battle subjectives (circumstance, policy, opinion, etc) your health will be the determining factor for your body’s ability to stave off attack. This goes hand-in-hand with making strong choices based on your personal circumstances. We call it Individual Risk Assessment. Walking through the studio doors means you’re comfortable with the inherent risk of the studio environment. People will touch things and gather which will bring some sort of risk. You are welcome to choose your involvement according to your individual risk assessment and choose to not hold Palisades Performing Arts & Fitness Studio, LLC liable for any air-borne sickness contracted while on the premises of 522 Palisades Dr. Pacific Palisades, CA 90272

I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance, acting, musical theatre, fitness and music activities. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Palisades Performing Arts & Fitness Studio, LLC and PH Village Partners, LLC ℅ Domino Realty, its’ officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as Palisades Performing Arts & Fitness Studio, LLC.

I hereby agree to release Palisades Performing Arts & Fitness Studio and PH Village Partners, LLC â„… Domino Realty and hold them harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in any activities on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury.

I have signed this document voluntarily and of my own free will in exchange for the privilege of participation. I also give Palisades Performing Arts & Fitness Studio, LLC permission to use my child’s picture in or on any form of advertisement or Palisades Performing Arts & Fitness Studio, LLC affiliated event.

If I am a minor, my parent and / or legal guardian has also signed this document releasing Palisades Performing Arts & Fitness Studio and PH Village Partners, LLC â„… Domino Realty from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.

I warrant the below information is complete and correct. I further release Palisades Performing Arts & Fitness Studio, LLC and PH Village Partners, LLC ℅ Domino Realty of all liabilities associated with my child’s attendance at studio location of 522 Palisades Dr., Pacific Palisades, CA 90272.
