I recognize the potential for injuries, which can occur in gymnastics and activities involving tumbling, trampolining and exercise. I hereby consent to the above guest participating in activities on equipment used by Skye High Gymnastics Center and hereby agree to release the CRAF Center, Skye High Gymnastics Center, it's officers, employees, teachers and coaches from all liability for any and all damages suffered by my child while under the instruction, supervision, or control of Skye High Gymnastics Center.

Effective for Fall 2022 Schedule:

All parties must enroll in auto pay. Tuition and fees may be paid via cash or check still, but it must be paid before the 1st of the month to avoid auto payment processing.

CONSENT: I consent to my/minor’s participation in the activity and acknowledge that I fully understand my/minor’s participation may involve risk of serious injury, illness, or death, including losses which may result not only from my/minor’s own actions, inactions or negligence, but also from the actions, inactions, or negligence of others, the condition of the facilities, equipment, or areas where the activity is being conducted, and/or the rules of play of this type of activity. I understand that if I have any risk concerns, I shall discuss them completely with the staff before I sign this agreement and before my/minor’s participation in the activity begins. Knowing and understanding the risks involved with participation in the activity, I hereby voluntarily and willingly assume full and complete responsibility for all losses and damages, including injury, illness, and death, resulting from my/minor’s participation in the activity, including transportation to and from the activity. I agree I am financially responsible for any losses and damages resulting from my/minor’s participation in the activity.

DISCLAIMER: Skye High Gymnastics Center, LLC (“Skye High Gymnastics”) is not responsible for any injury (or loss of property) to any person while practicing, training, taking class, competing, participating in open gym, special events, demonstrations or shows, or in any other way involved in gymnastics, cheerleading, or teams (the “activity”) at Skye High Gymnastics for any reason whatsoever, including ordinary negligence on the part of Skye High Gymnastics, its members, managers, agents, or employees.

PHOTO & VIDEO RELEASE: I grant consent for my/minor’s picture to be taken or to be filmed while participating in activities at Skye High Gymnastics. I authorize Skye High Gymnastics to use and publish images, photographs, pictures, portraits, and audio, video and/or film footage of me/minor in all forms of media and in all manner for publication including, but not limited to, advertising and marketing campaigns, press releases, periodicals, and website use. I hereby waive any right I may have to review, inspect, edit or approve such publication and I release Skye High Gymnastics from any claims I may have against it for use of such images, photographs, pictures, portraits, and audio, video and/or film footage of me. I affirm that I am of legal age and am freely agreeing to this clause.

Welcome to Skye High Gymnastics Center!

OUR MISSION
OUR MISSION IS TO ENCOURAGE PARTICIPATION IN ALL ASPECTS OF GYMNASTICS, SUPPORT ATHLETES IN THEIR PURSUIT OF COMPETITIVE EXCELLENCE, GENERATE PUBLIC AWARENESS OF THE SPORT OF GYMNASTICS, AND HAVE EXCEPTIONAL COMMITMENT TO CUSTOMER SERVICE THROUGHOUT THE PROGRAM.

CLASS ATTIRE/GENERAL RULES
Students must come to class wearing tight, stretchy clothing. Leotards are the most optimal, but leggings or spandex shorts are acceptable.
Jeans or pajama pants are unacceptable.
Hair must be tightly pulled back BEFORE class begins.
Shoes must be removed before entering the gym.
No gum is allowed in the gym.
Water bottles are allowed in the gym. No other food or drink is allowed.
Students must ask for permission before leaving the gym.
Parents are not allowed to talk to the students while they are in the gym.
Parents are able to watch from the windows and doorways. THEY ARE NOT TO COME IN UNLESS INSTRUCTED BY A COACH.
If a student is disobeying rules, improperly using equipment, or disrespecting coaches or other athletes, they will be asked to leave the room.

CLASS COSTS/FEES
Each monthly payment will be the same, averaging four classes per month.
Payment is due the first class of the month. Any payment made on or after the third week of the month will be charged a $15.00 late fee. If payment is not made by the third week of the month, your child will not be able to participate in class.
Tuition is non-refundable. Autopay only charges tuition and will not charge additional fees added to your account.

CLASS PLACEMENT
All students will begin in an age-level based class. If their skill level exceeds the class, they will be asked to move to a more advanced class by the coach’s choice.

CLASS CANCELLATIONS
If class is canceled due to weather or instructor illness, an email will be sent to the email you registered your child with. Coaches try to announce a canceled class by 2PM that day. A make-up date/time will be posted as well. Refunds will not be given for class canceled due to weather. It is out of the gym's control, and we do our best to provide a make-up class that works for everyone.

PARENT/COACH COMMUNICATION AND CONTACT INFORMATION
If there is a need for a parent to communicate with a coach, please email them to first discuss the problem. Our coaches are very busy, and we cannot take time away from the other students to talk with parents.
Please come to any of your child’s coaches if you have a problem or concern, or have general questions. Your thoughts are important to us, but we can’t know what you need or are thinking unless you talk to us. If we cannot help you we will explain why and we will work to find another way to resolve the issue.

WAIVER: In consideration for my/minor’s participation in the activity, I hereby waive all claims or causes of action, including ordinary negligence, against Skye High Gymnastics, its managers and members, and any of their employees, teachers, coaches, or agents, arising out of my/minor’s participation in the activity wherever, whenever or however the same may occur. I understand that this waiver is intended to be as broad and as inclusive as permitted by the laws of the State of Michigan and agree that if any portion is held invalid, the remainder of the waiver will continue in full legal force and effect. I further agree that the venue for any legal proceedings shall be within the State of Michigan. I have read this form and fully understand that by checking this box, I am giving up legal rights and or remedies which may be available to me/minor for the ordinary negligence of Skye High Gymnastics or any person listed above.
