I have received or will obtain a copy of A Step Ahead Dance - ASADC, LLC policies and acknowledge that I
am responsible for carefully reading and following the rules and policies therein. I understand that A
Step Ahead Dance – ASADC, LLC does not give credit and/or refunds for class(es) missed due to holiday,
vacation, illness, weather, etc. I further understand that there are specific risks of physical or property
damages, losses, or injury that may result from my or my child’s participation with A Step Ahead Dance
Center, and I voluntarily assume all the risks associated with such participation. By signing this release
form, I/we (the dancer and parent/guardian):
1. I understand and acknowledge that dance may require strenuous physical activity and that there is
a risk of personal injury or other losses or damages arising out of such.
2. I certify that the student is physically fit and has no physical or mental condition, which may limit
his or her ability to safely participate in the classes in which the student has enrolled, and/ or else I am
willing to assume, and bear the costs of all risks that may be created, directly or indirectly, by any such
condition.
3. I hereby acknowledge and agree that, by participating in the dance programs offered, I assume all
of the risk of injury, and I hereby agree further that I will not assert any claim against A Step Ahead
Dance - ASADC, LLC, its employees, officers, directors or members or any of its subsidiaries or affiliated
companies by reason of any injury, or other losses or damages arising out of the student's participation
in the dance classes and performances.
4. I hereby voluntarily release, and agree to indemnify and hold harmless A Step Ahead Dance -
ASADC, LLC, its employees, agents, officers, directors and members or any of its subsidiaries or affiliated
companies, from any and all claims, demands, or causes of action, which are in any way connected with
the student's participation in the dance classes or recitals and any and all liability for any injuries or
illnesses sustained or incurred while participating in the program at A Step Ahead Dance - ASADC, LLC or
during any activity organized by A Step Ahead Dance - ASADC, LLC.
5. Should A Step Ahead Dance - ASADC, LLC, or any of its subsidiaries or affiliated companies, or
anyone acting on their behalf, be required to incur attorney's fees and costs to enforce this agreement, I
agree to indemnify and hold them harmless for all such fees and costs.
6. I hereby authorize the staff members at A Step Ahead Dance - ASADC, LLC to act for the student in
any emergency requiring medical attention using their best judgment. ​
7. All medical expenses incurred will be the responsibility of the student or the student's family. I have
no knowledge of any physical or mental impairment or disability that would prevent participation in this
program, as outlined in the waiver, which I have read.
8. The student is covered exclusively by medical and other health insurance, and I am responsible for
all medical payments.
9. I acknowledge that by registering for A Step Ahead Dance - ASADC's Main Session that I will pay
monthly tuition installments for September through May and that I will pay all costume/show fees
associated with the session. I further agree that students may only be withdrawn from class with 30
days written notice and items billed to the account, including, but not limited to tuition, costumes,
rehearsal fees, before the withdrawal date must be paid in full. If the balance is not paid within 30 days
of the dancer's last class, I understand that my account may be put into collection and I agree to pay all
additional fees incurred by the studio.
10. I acknowledge that a late fee will be billed to my account if tuition is not paid on the first of each
month.
11. I acknowledge that in registering for A Step Ahead Dance's Main Session that I will pay the costs
associated with a Spring Performance for my dancer. Costume fees range from $90-$130 per class and
there will be a $27 per class rehearsal fee. Costume deposits will be due at the time of registration.
Costume balances and Rehearsal Fees will be due on November 1st.
Photography Release - Students are sometimes included in public relations pictures for release to the
public in press or on our website (www.astepaheaddancecenter.com). A Step Ahead Dance - ASADC, LLC
may occasionally use (except where prohibited by law) the students name, likeness, biographical
information such as age and town where the student lives, photograph, and any statement or remark the
student or his or her parent has made about participating in our dance programs or recitals for
advertising and promotional purposes without additional compensations.
I give permission for A Step Ahead Dance - ASADC, LLC to use the student's photographs and name,
biographical information, or statements.
