Waiver of Liability
(Required to take classes)

As a participant in any program or class of A Time to Dance, LLC, I recognize and acknowledge there are certain risks of physical injury, and I agree to assume the full risk of injuries, including death, damages or loss that I or my minor child may sustain as a result of participating in any activities connected with or associated with such a program or class. I understand that an inherent risk of exposure to infectious disease exists in any public place where people are present. By registering for dance and willingly entering A Time to Dance’s studio space, I voluntarily assume all risks related to exposure to any infectious disease.

I agree to waive and relinquish all claims I or my minor child may have as a result of participating in any program or class against A Time to Dance, LLC and its officers, agents, servants, employees and independent contractors.

I further agree to indemnify and hold harmless and defend A Time to Dance, LLC and its officers, agents, servants, employees and independent contractors from my claims resulting from injuries including death, damages and losses sustained by me or my minor child that arise out of, in connection with, or in any way associated with the activities of this program.

I HAVE READ AND FULLY UNDERSTAND THE ABOVE WAIVER AND RELEASE OF ALL.

Please agree to this waiver if you will permit use of your/your child’s image as described below. If you do not sign this waiver, we will not use your/your child’s image as described.

I understand that A Time to Dance, LLC is an organization that promotes dance as an art form not only to its students, but also to the community at large. As such, articles, brochures, videos and websites may be used either promotionally or educationally and may include images of A Time to Dance, LLC’s students or other participants in our programs.

I hereby permit the use of my or my daughter/son’s image to be photographed, videotaped or otherwise recorded for use in A Time to Dance, LLC’s publicity or educational materials. These materials include, but are not limited to, photographs of classes and performances, newsletters, various other school publications, A Time to Dance, LLC’s internet webpage page, and videos of classes or performances.

I agree to pay nine months (September - May) of tuition for all classes and all of the associated fees. I agree that if I choose to leave the classes or change classes, I will need to do so by November 1. Otherwise, I will pay the tuition and associated fees for the above classes for the nine months of the dance season. In the event of online class availability only, the studio will work with each individual family to remedy any tuition issues due to lack of accessibility. Please know that only so many students are allowed in each class and that any termination after November 1, will incur a $100 early termination fee.

_______I have received and understand the contents of the A Time to Dance Studio Handbook including financial obligations, Behavior Policies, Infectious Disease Policy. By signing this Studio Family Agreement you agree to inform your dancers of the studio expectations and to follow up any questions with studio staff or Mrs. Dianna.

_________Attending class each week is very important. Students learn routines and formations based on how many students are in their class. When one student is out, it affects every girl in class. For this reason attendance is highly stressed. If a class is cancelled on the instructor’s behalf, the class will be rescheduled for a later date. In the event we are unable to reschedule, the amount of that lesson will be credited to the students’ account. This policy does not apply to cancellations due to weather or school district closure. However, if a class is missed by a student no refund, credit, or make up class is offered. If a student is out for an entire month, the $45 registration fee must be paid to rejoin the class. Dance is a yearlong commitment and all fees and tuition are non-refundable. If a student falls behind due to poor attendance, private lessons can or will be scheduled at the student’s expense.
