I have enrolled my child/myself in a program of physical activity including, but not limited to ballet, tap, jazz, pointe, musical theater, vocal, art lessons, and camps offered by Pulaski Performing Arts Center. I hereby affirm that my child/I is/am in good physical condition and does/do not suffer from any disability which would prevent or limit him/her/myself from participating in any of the above mentioned activities.
In consideration of my child's/my participation in Pulaski Performing Arts Center's activities and classes I, for myself, my heirs and assigns, hereby release Pulaski Performing Arts Center, its owners, owner's family, employees, teachers, assistant teachers, student teachers, and students from any claims, demands and causes of action arising from my child's/my participation in Pulaski Performing Arts Center programs and outside activities. In addition, I release any facility, school, theater, auditorium church, the City of Pulaski or any venue used by PPAC during performances, from legal action should my child/myself come to harm while participating in PPAC related events.
I fully understand that my child/I may injure myself as a result of his/her/my participation in Pulaski Performing Arts Center's programs and I hereby release Pulaski Performing Arts Center from any liability now or in the future including, but not limited to heart attacks, asthma attacks, muscle strains, muscle pulls or tears, broken bones, shin splints, heat prostration, concussions, or any other injuries to the body, illnesses and soreness or injury however caused, occurring during, or after my child's/my participation in classes and events including, but not limited to camps, recitals, parades, outside performances, competitions, and conventions.at and related to Pulaski Performing Arts Center. I hereby affirm that I have read and fully understand the Waiver of Liability and Consent and that I am the legal parent/guardian of the child I am registering and that I am an adult at least 18 years of age

I agree to allow Pulaski Performing Arts Center to photograph or videotape me or my chilldren at any Pulaski Performing Arts Center event related to the use of any and all spaces used by Pulaski Performing Arts Center for the use of advertising and literature on paper or social media for Pulaski Performing Arts Center. This is included, but not limited to, in the studio, recital, competitions, workshops, conventions, street performances, and fundraisers. No compensation will be allotted for the use of these photographs or videos.

Furthermore, I agree that any photo or video taken by me, the parent/legal guardian, at a Pulaski Performing Arts Center event must have the permission of the parents of the students in the photo or video to be published on any type of social media.

I also agee that videos of choreography produced by Pulaski Performing Arts Center that are taken by me and my family (including my enrolled student) will not be published on social media or YouTube without the express permission of all dancers involved in the video and without the permission from the choreographer and studio owner.
