In consideration of acceptance of this registration for classes, or my child(ren) being permitted to take part in this event, I agree as follows:

1. To save harmless and keep indemnified Andrew and Christina Sjwed, their studio, their instructors, servants and representatives from and against all claims, actions, costs and expenses and demands in respect of death, injury, illness, loss or damage to my or my child(ren)'s person, however caused, rising out or in conjunction with my or my child(ren) taking part in this studio's classes/events but limited to taking part in this event and not withstanding that the same may have been contributed to or occasioned by the negligence of Andrew or Christina Sjwed, their studio, their instructors, servants and representatives.

2. Andrew and Christina Sjwed operating as Andy and Tina's Dance FX are not responsible for items that are lost or stolen from this studio's premises/events.

3. I realize that a costume is required for every choreography class. Once my costume is started, even upon withdrawing from that class, I will be responsible for the full amount of the costume. I am aware of the price guideline as outlined on the website and acknowledge that I will be responsible for paying the same.

4. I realize that festival/competition fees are required for every competitive class. Once my entry fees have been paid on my behalf, even upon withdrawing from that class, I will be responsible for the full amount of those fees.

5. I understand that an interest charge (2% per month) will be applied to all amounts 90 days past due and acknowledge that I will be responsible for paying them in full by the season's end.

6. Andy and Tina's Dance FX has my permission to use my or my child(ren)'s name and/or photo for advertising or publication purposes.
