RELEASE AGREEMENT: Acceptance of this waiver is without assumption of responsibility of any kind by the Dekeuster’s program for injuries or losses which may be suffered or sustained in connection with activities during the duration of your child's attendance at Dekeuster's Gymnastics. All participants are expected to carry their own accident and/or medical insurance. Coaches and Instructors of the Dekeuster’s program are safety conscious and follow appropriate safety procedures. In the event of injury or illness, every effort will be made to contact the parents or guardian. If necessary, I authorize Dekeuster’s Gymnastics to administer first aid and/or medical treatment.

I, hereby consent to the photographing and video recording of myself/my child, by Dekeusters Gymnastics, during participation in gymnastics activities and related events. I understand that these photographs and videos may be used by Dekeusters Gymnastics for promotional purposes, including but not limited to posting on the organization's website, social media platforms, promotional materials, and other marketing channels.

I understand and agree that Dekeusters shall have the right to use, reproduce, display, distribute, and publish the photographs and videos without any compensation to me or my child. I waive any right to inspect or approve the finished product or its use.

I hereby release, discharge, and hold harmless Dekeusters Gymnastics, its officers, directors, employees, agents, representatives, and volunteers from any and all claims, demands, actions, or liabilities arising out of or in connection with the use of the photographs and videos, including but not limited to any claims for invasion of privacy, right of publicity, defamation, or copyright infringement.
