ADSD 2023-2024 School Year
POLICY
I, the parent/guardian of the participant, have agreed to join ADSD for the full school year, September - June.
• I, the enrolled participant and the parent/guardian, understand that students are expected to conduct themselves properly while at the studio and when at a performance event. Any unnecessary talking or disruptive behavior in class will not be tolerated. Misrepresentation or slander of ADSD or its students can result in expulsion.
• I, the enrolled participant and the parent/guardian, understand that tuition payments, registration fees, and recital fees will be made on time. A one-time registration fee of $25.00 ($15.00 discount if register early) will be paid and is nonrefundable for any reason. Any tuition account paid after the 5th of each month will be assessed a late fee of $15.00. All returned checks will incur a $10.00 fee. A student with an overdue balance of more than 60 days will NOT be allowed into class or to perform until payment in fully is received. Costume payments must be paid on time or will result in a $25.00, or more, separate shipping fee. Costumes will become property of ADSD if not properly paid for or if left at the studio after 90 days.
• Beginning October 1, 2023, I, the parent/guardian of the enrolled participant, commit to the full 2023-2024 dance season and the monthly tuition payments. Only under Director approval may I waive this contract.
• I, the enrolled participant and the parent/guardian, understand that all routines belong to ADSD and will not perform, teach, or upload videos of routines online without permission from the director. I will not perform my own choreography in public without Director acknowledgement. You are a representative of ADSD.
• I, the enrolled participant and the parent/guardian, will read over and keep up with the ADSD handbook.

LIABILITY/WAIVER RELEASE FORM - ADSD
Please read carefully before signing. This is a release liability and waiver of certain legal rights.
I, the enrolled participant and/or the parent/guardian of the participant agree and understand that dance/fitness training is a potentially hazardous activity. I recognize that there are certain risks inherent in dance training including but not limited to serious physical injury. The participant hereby agrees to participate in activities of the Angela Davis School of Dance and hereby agrees to indemnify and hold harmless ADSD and its instructors against any liability resulting from injury that may occur to the participant while participating in activities of the Angela Davis School of Dance. The participant also agrees to indemnify ADSD for any damages incurred arising from any claims, demand, action or course of action by the participant.
The participant authorizes any representatives of ADSD to have the participant treated in any medical emergency during their participation in activities of the Angela Davis School of Dance. Further, the participant and/or parent/guardian agrees to pay all costs associated with medical care and transportation for the participant.
Any special medical/health problems of which the staff should be aware are attached to this form.
I HAVE CAREFULLY READ THE ABOVE LIABILITY RELEASE AND SIGN IT WITH FULL KNOWLEDGE OF ITS CONTENT AND SIGNIFICANCE. ADDITIONALLY, I HAVE READ AND AGREE TO ABIDE BY ALL ENCLOSED POLICIES AND PROCEDURES.
PHOTOGRAPH/LIKENESS RELEASE FORM
We (I), the parent(s)/legal guardian(s) authorize the Angela Davis School of Dance and/or its representative, agent, or employee to use any photograph / likeness of my minor child, for any purpose, including publicity, choreographic archives, promotional materials and/or any other reason deemed appropriate by the Angela Davis School of Dance.
OR
I, age eighteen (18) or over (no longer a minor), authorize the Angela Davis School of Dance and/or its representative, agent, or employee to use any photograph / likeness of me, for any purpose, including publicity, choreographic archives, promotional materials and/or any other reason deemed appropriate by the Angela Davis School of Dance.
