You must be over 18 to register for an account. You must also be the legal guardian of any minor students added to your account. You must be the authorized user of the credit card that is associated with your account.

Registration Form, Participation Agreement & Release of Liability

The undersigned, individually or as parent or guardian of the registered student(s), as well as my heirs, executors, agents, assigns, and representatives, hereby indemnify, release, and forever hold harmless High Impact Dance, its owners, directors, employees, instructors, and volunteers from any and all liability arising from any accident, personal injury, sickness, natural disaster, death, or property loss or damage sustained by my child/myself/the minor child for whom I am a legal guardian, or any guests that I may bring with me, while that person (hereafter “Participant”) is a spectator, attendee, or participating in activities connected with High Impact Dance, including but not limited to dance classes, rehearsals, performances, virtual classes, parties, camps or other activities. I understand that dance activities may have risks of injury, and being fully aware of those risks, I consent to the Participant participating in the programs and activities offered by High Impact Dance.

I accept full responsibility and liability for any and all necessary medical and accidental injury treatment of the Participant and give my permission for them to receive onsite medical care and to be transported or cared for by emergency service responders if necessary. I also accept full responsibility for any costs associated with the medical care or transport. By agreeing to this statement, I declare that the Participant is in good health, with no physical conditions that might prevent his/her/my participation in dance classes, rehearsals, performances, parties, camps, or other activities at or under the direction of High Impact Dance.

I understand and acknowledge that because of the physical nature of dance, there may be physical contact between instructors and students during classes or other activities. I understand that at times, for proper instruction and safety, physical contact is required, necessary, and authorized hereby.

I further acknowledge, understand, appreciate, and agree that my or the Participant's participation may result in possible exposure to illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation and exposure.

I acknowledge and agree that High Impact Dance is not responsible for the Participant before or after their scheduled class time or any other event and that Children shall not be left unattended by me while in the waiting room or otherwise. I also acknowledge and agree to pay the $15 late pickup fee, if I am not at the entrance to pick up my child when class is dismissed.

I have read the High Impact Dance "Dress Code" and the corresponding season "Tuition and Studio Info" and agree that myself, my student, and any other persons, that are with me will follow them. I will discuss them with the aforementioned persons and make sure they have a clear understanding of what is expected.

I give my permission for the Participant to be included in pictures and videos while in the studio or at an event. Those pictures and videos may be posted on the High Impact Dance website and social media, used for teaching purposes, and used on other promotional materials, at the discretion of High Impact Dance.

I have carefully read this Registration and Release, fully understand the contents and agree that this is an assumption of risk by the Participant and release of liability to High Impact Dance, which is agreed, authorized, and accepted of my own free will. I agree to comply with the policies and procedures of High Impact Dance. I fully understand that failure to comply with all such policies and procedures may result in the immediate suspension or dismissal of the Participant. I agree that I will conduct myself in a positive and appropriate manner while on the premises of High Impact Dance or at any of its events. I accept full financial responsibility for any charges the Participant incurs at High Impact Dance or at any of their events. I agree that a withdrawal form must be completed and submitted to High Impact Dance at least 2 weeks before withdrawing a Participant from any class, or a full month of tuition for the next month or session will be charged. A $25 cancellation fee will apply when dropping a class, or lab, or camp within 14 days of the start date unless otherwise stated in the tuition policy for that specific season or class.
