Must be 18 years or older to register for an account

* The Registration Fee and Tuition are non-refundable.
* Tuition is due by the first of every month. Tuition is considered LATE after the seventh day of the month, and a $15.00 late fee (per month) will be added to the total amount due.
* If billing is necessary, an additional $5.00 processing fee will be charged.
* There is a $30.00 charge for RETURNED CHECKS & a $10.00 charge for DECLINED CARDS.
* CANCELLATIONS: WRITTEN notice must be given to the office by the 25th of the month or you are responsible for the next month's tuition. Withdrawal MUST be done in person and will NOT be accepted over the phone. You must complete a withdrawal form in the office.
* A $10.00 re-registration fee will be charged when student drops and returns.
* Tuition will NOT be pro-rated for absences. If student misses a class, she/he has 30 days from that date to make it up. If a class is removed from your schedule, she/he will forfeit any 'make-ups' accumulated up to that point, from said class.
* By signing below, I understand, accept, and agree to abide by the RJAD Inc.'s 'fees and tuition' policies stated above.

The enrolled student, or the parent or legal guardian of the minor child who is registering for classes, hereby acknowledges that the classes, practices, performances, transportation to and from, and all activities incidental thereto, in which I am enrolling myself or said child, might result in illness, physical bodily injury and/or psychological injury occurring to myself or said child. With this understanding, I nevertheless consent to my or the child's enrollment in the RJAD Inc.'s Academy of Dance Program of activities and classes.
In consideration of Jennifer Hatfield & Rachel White, the RJAD Inc.'s Academy of Dance, and all authorized teachers, assistants and others connected therewith, enrolling myself or a child, I agree on behalf of myself, my spouse, if any, my child and all others, that each of the foregoing and all their agents and associates shall be and hereby are fully released from any claims, damages, fees, costs, or expenses, and of every liability whatsoever which may arise, result from or be related to personal, physical or psychological injury of any kind to myself or the enrolled child. I understand hands on correcting will at times be necessary to prevent injuries and ensure proper training.
*I further agree to indemnify and hold harmless all of the above-named parties from liability, claims, damages, fees, costs and expenses, including attorney's fees and court costs, of any kind whatsoever, which may occur during or in connection with the activities enumerated above, as may pertain to myself or the enrolled child.

* I consent to the use and reproduction of any studio photographs of myself or minor child to be used by the RJAD Inc.'s
Academy of Dance for advertising purposes.
* I consent to pictures of said enrolled student to be posted on social media.
