RELEASE OF LIABILITY
By registering for classes, I understand that classroom participation at Ovation School for the Performing Arts could include actions or tasks which might be hazardous to the participant. I assume any risk of harm or injury which might occur to the participant due to
his/her participation in the events or activities sponsored by Ovation School for the Performing Arts. I release the organization or business named above from all liability, costs and damages which might arise from participation in these events or activities. I agree that my minor has my consent to participate in these activities or events. I further provide my consent for the organization or business named above to seek emergency treatment for the minor if necessary. I agree to accept financial responsibility for the costs related to this emergency treatment.

PHOTO/VIDEO RELEASE
By registering for classes, I consent to the use of my student’s name and likeness, including his/her performance, voice and image in any form, to be used for promotional purposes of Ovation School for the Performing Arts, WITHOUT COMPENSATION or further permission or notification Ovation School for the Performing Arts. I hereby waive any inspection or approval of use.
