Assumption of the Risk and Waiver of Liability
Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, limited the congregation of groups of people.
Roanoke Ballet Theatre (RBT) has put in place preventative measures to reduce the spread of COVID-19; however, the RBT cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending classes at RBT could increase your risk and your child(ren)’s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by coming to RBT and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at RBT may result from the actions, omissions, or negligence of myself and others, including, but not limited to, RBT employees, RBT company members, and students of the school and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at RBT or participation in RBT performances and events (“Claims”).
On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless RBT, its employees, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of RBT, its employees, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any RBT classes, performance, or events.

• All accounts must be on auto payment. Auto payment for monthly tuition is not optional.
• Tuition is posted to accounts on the DanceStudio-Pro registration site on the 1st of each month.
• Payment of tuition, if you choose to pay before the due date, is due by the 5th of the month.
• Auto payments are charged on the 5th of the month. If the 5th of the month occurs on a Sunday, holiday, or a day when RBT is not in session, payment is due on the following work day.
• There is a $35 late fee for unpaid monthly tuition on the 6th of each month. The financially responsible parties must keep credit card information and contact information up to date. If a credit card is declined, there will be a $35 late fee added to the existing tuition each month the card continues to be declined.
• Parents are financially committed to the first 3 months of payment for classes. Your card will be auto-drafted on the registration site.
• If there is a wish to terminate this contract / drop from all classes, a WRITTEN notification of intent (email to rbtoffice@roanokeballet.org or letter addressed to Sandra Meythaler) to terminate the contract must be received by 1 month prior to the student’s last day. The financially responsible party is still obligated to fulfill their tuition commitment through the end of the first 3 months (mentioned above).
• If the financially responsible parties choose to pay YEARLY, they are responsible for payment for the entire year, in one payment.
• Yearly payments are due on September 5th and can only be chosen as a payment option before September 5th.
• Tuition for your first month is due at time of registration. If the season has already begun, you will need to register in the month you plan to start classes in, prior to the beginning of the class. If the season has not yet begun, you may register before that month.
• All students must be registered for the class. “Pay as you go” classes are not offered.
• Tuition is non-refundable. Missed classes, including inclement weather-related absences, global pandemics, and days when RBT is not in session, may be made up in equal dollar value classes within the same year.
• If RBT is mandated to be closed due to COVID or any other pandemic situation, all classes will resume on Zoom. Tuition is non-refundable and we will follow the same cancellation policy as stated above.
• All scholarship students are responsible for paying the full tuition amount for August (or their first month), registration fees, and production fees.
• Roanoke Ballet Theatre assumes no liability for accidents or injuries sustained during classes, rehearsals, performances, transportation to or from activities, or waiting before/after classes.
• Roanoke Ballet Theatre’s mode of communication is through email, Facebook, and Instagram. It is the student/parent/guardian’s responsibility to read all email communication. If you have any issues or concerns, you may email the RBT office email or the Director, Sandra Meythaler, directly at smeythaler@roanokeballet.org.
• Students must be picked up no later than 5 minutes past the ending of their class/rehearsals and must not arrive any earlier than 20 minutes prior.
• Street shoes will not be allowed on the dance floors; only approved footwear for classes.
• In an effort to keep the studio clean, students are required to clean up after themselves (ex.- snack wrappers, spilled food/drinks, toe tape/band-aids).
• Students are responsible for all of their belongings. RBT is not responsible for lost or stolen items. Please keep all valuables (ex.- jewelry, electronics) at home.
• I give Roanoke Ballet Theatre permission to use photographs, video, or artistic renderings of myself/my child for promotional or fundraising use.
