Sandra Lynn's School of Dance is the home of Dancing, Theatre, and Yoga. By accepting this, I agree with the policies for the classes that I will be attending. There will be a copy of the waiver below for the classes you sign up for in your parent portal, underneath the waivers icon.
A copy of Sandra Lynn's School of Dance policies will be in the shared files icon, and I will take the responsibility to carefully read and follow the rules and policies therein.
Students of Sandra Lynn's School of Dance may have the opportunity to be photographed or videotaped at various dance events. Sandra Lynn's School of Dance and other Sandra Lynn's School of Dance members are hereby granted permission to take photographs and video of the students to use in social media, brochures, posters, advertisements, and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name. The release will allow us to videotape our Showcase as well as record in-class footage to share videos for students to rehearse choreography. These photos or videos may be included for Sandra Lynn's School of Dance publications or advertisements, including on our website and various social media sites. No financial compensation shall be given for the use of any photographs or videos. I acknowledge that Sandra Lynn's School of Dance may choose not to use my photo or video at this time but may do so at its own discretion at a later date. Sandra Lynn's School of Dance reserves the right to discontinue the use of photos or videos without notice. I understand that in some cases, my child/children and/or my name may be used. I have read this release and I understand its contents. I consent to and authorize the use by Sandra Lynn's School of Dance or anyone authorized by Sandra Lynn's School of Dance of any and all photographs or videos that have been taken of my child/children/myself. If you do not want your child to accept the public photo waiver, please go to your waiver icon and click photo opt-out to follow the instructions. However, all rehearsal videos posted to band will be of the entire class and are on a private SLSD account.
No REFUNDS on tuition and/or missed classes. A $35.00 fee for all NSF checks returned and any ACH returns of Merchant Activity and $6 for returned CC. If any legal/collection counsel is engaged, the Guardian/Adult is responsible for their legal representation/fees and any SLSD legal/collection fees and all administrative fees.
Sandra Lynn's School of Dance, LLC, does not carry medical insurance for its members. It is required that all members be covered by their own family insurance policies, and if an injury occurs, it is understood that the artist's policy is your only source of reimbursement. I hereby agree and accept.

I agree to the following: Sandra Lynn's School of Dance (hereinafter sometimes referred to as Sandra Lynn's) is, in no way, responsible for the safekeeping of my personal belongings while I attend class. I understand that Sandra Lynn's offers Tap, Jazz, Ballet, Acro, Lyrical, Contemporary, and Hip Hop classes. I voluntarily participate in the classes at Sandra Lynn's with the full knowledge that there is a risk of personal injury, property loss, or death. I will assume full responsibility for any risks, injuries, or damages, known or unknown, that I might incur as a result of participating in the program at Sandra Lynn's. I agree that neither I, my heirs, assigns, nor legal representatives will sue or make any other claims of any kind whatsoever against Sandra Lynn's School of Dance, LLC, its teachers, or its members for any personal injury, property damage or loss, or wrongful death, whether caused by negligence or otherwise. I release Sandra Lynn's School of Dance and its instructors from liability for injuries arising out of my practice at the studios or at off-site events, arising out of my use of its facilities, or both. Please add all injuries/physical limitations/conditions to your student profile. All students will be backstage for the entire recital performance. No parents are permitted backstage unless they are a cleared parent helper. Payment is required for the entire session, even if the student decides to quit mid-session. A $5.00 Fee will be applied to your account for each outstanding balance after 14 days.

I agree that neither I, my heirs, assigns, nor legal representatives will sue or make any other claims of any kind whatsoever against Sandra Lynn's School of Dance, LLC, its teachers, or its members for any personal injury, property damage or loss, or wrongful death, whether caused by negligence or otherwise. I agree to the following: Sandra Lynn's School of Dance (hereinafter sometimes referred to as Sandra Lynn's) is, in no way, responsible for the safekeeping of my personal belongings while I attend class. I understand that Sandra Lynn's offers yoga classes and instruction services in an arduous, physical style of yoga practiced for durations of 60-75 minutes in a lightly heated (ranging from approximately 75-88 degrees) room. I understand that classes at Sandra Lynn's may be physically strenuous and pose health risks, and I voluntarily participate in classes at Sandra Lynn's with the full knowledge that there is a risk of personal injury, property loss, or death. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in all yoga classes at Sandra Lynn's. I represent and warrant that I am physically fit and have no medical conditions that would prevent my full participation in classes at Sandra Lynns. I understand that some conditions may compromise my safe practice of yoga, including, for example, but not limited to: pregnancy, heart disease, high or low blood pressure, broken bones, drug prescription, asthma, and sprained muscles. Within the previous 6 months, my doctor has evaluated my physical condition and cleared me to practice yoga at Sandra Lynns. Before each yoga class, I will drink plenty of water, refrain from ingesting narcotics, drugs, or alcohol; avoid big meals 2-3 hours before class; and avoid class if fasting. I will additionally disclose potentially compromising injuries or conditions I have to the instructor. I will assume full responsibility for any risks, injuries, or damages, known or unknown, that I might incur as a result of participating in the program at Sandra Lynn's. I agree that neither I, my heirs, assigns, nor legal representatives will sue or make any other claims of any kind whatsoever against Sandra Lynn's School of Dance, LLC, its teachers, or its members for any personal injury, property damage or loss, or wrongful death, whether caused by negligence or otherwise.
