MEDIA I grant Victory Arts Center under Victorious Visions LLC the right to use my image, voice, appearance and/or performance as captured by photographic, audio and/or video means (the “Product”)for release and/or reproduction in any media including but not limited to illustration, promotion, art, editorial and advertising.

LIABILITY I understand that there is risk of physical injury associated with, arising out of an inherent to the activity of dance movement. In recognition of this acknowledge risk of Injury, I knowingly and voluntarily wave all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Victory Arts Center & Victorious Visions LLC, it’s officers, agents, employees, instructors, subsidiaries, corporations, and all affiliated entities. I hereby agree to release Victory Arts Center of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risk of physical injury arising out of active participation in Dance Camp on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
