ENGLISH THEATRE ARTS WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19

ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

The Undersigned hereby states and agrees as follows:

The Undersigned fully understands and assumes any and all risks associated with participating in any of English Theatre Arts classes, workshops, and related events which may result in exposure to and/or illness pertaining to communicable diseases including but not limited to SARS, MERS, MRSA, Influenza, and COVID-19.
The Undersigned agrees to fully cooperate and comply with any and all safety procedures and protocols put in place by English Theatre Arts (ETA), their Instructors, Staff, Sub-Contractors, Employees, and Agents. Said procedures and protocols are hereby attached and incorporated herein.
The Undersigned, his/her/their agents, heirs, successors and assigns, hereby agrees to INDEMNIFY, RELEASE, AND HOLD HARMLESS English Theatre Arts, their instructors, staff, sub-contractors, tenants, officers, agents, property owner(s), and their heirs, successors, and assigns, from any and all liability, claims, actions, or causes of actions as pertains to any and all communicable disease and COVID-19.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IF FREELY AND VOLUNTARILY.



FOR PARTICIPANTS UNDER AGE 18 AT THE TIME OF REGISTRATION

I, as Parent and/or Legal Guardian, have read and fully understand and acknowledge the above Assumption of Risk/Waiver of Liability/Indemnification Agreement and represent that I have full authority to sign on behalf of my child(ren) and that my signature binds each other person having authority to make decisions on behalf of the child(ren).



If your dancer, named above, test positive for COVID-19, we request you inform someone on staff with ETA. With your permission from the signature on this page, we will share that there has been a positive test, we will not release the name of the dancer.

We reserve the right to ask everyone in the studio to wear a mask if we feel that circumstances are such, that for health and safety reasons it becomes necessary.

I have received or will obtain a copy of English Theatre Arts policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that English Theatre Arts does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with English Theatre Arts, and I voluntarily assume the risks associated with such participation. I also give permission to English Theatre Arts to use any and all photography, video, and other media at their discretion.

English Theatre Arts has a no refund policy. Tuition and all other fees for any season at English Theatre Arts, including Registration Fees, Costume payments, Competition fees, Intensives, Workshops, Master Class fees, etc., are non-refundable once paid.
