Each Müv student may decline to participate in any activity and must inform any Müv instructor or staff of any physical limitations they may have. Please consult your physician of any injuries prior to participating in any Müv Dance & Fitness studio performances or class activities. I, the undersigned, represent that I am physically able to participate in all Müv activities. I hereby authorize any and all medical attention to be administered to myself or child, in the event of accident, injury, sickness etc., under the direction of Müv Dance & Fitness Studio, until such time as I or the appointed emergency contact person may be contacted. I assume the responsibility for the payment of any such emergency treatment. I have notified Müv Dance & Fitness Studio of any special medical needs or information required for myself or child. I, the undersigned, for myself, my child, my spouse, heirs, legal representatives and assigns, expressly release, waive, discharge and hold harmless Müv Dance & Fitness Studio, its directors, staff, employees, teachers, agents, successors, predecessors, sponsors, legal representatives and assigns from all claims, demands, losses, actions, judgments, suits, executions and liabilities of any kind. In addition, this release is for the entire premises of Müv Dance & Fitness studio, included but not limited to common areas, any location a Müv Dance & Fitness studio may be held. Participants over 18 years and over: I hereby represent that I am at least eighteen (18) years of age, understand and agree to the terms, conditions, and policies of Müv Dance & Fitness studio. Participants under 18 years: I, the undersigned, am the legal parent/guardian of the registrant and agree to the terms, condition and policies of the Müv Dance & Fitness Studio.

To drop a class, I understand that I must give Müv a 30 day notice to drop any class(es) by emailing the studio at info@muv678.com. Registration fees are non-refundable.

To drop a class date, you must email us directly at info@muv678.com with the class name, date and time, 24 hours before class time. Your class card will be punched otherwise. Please keep this in mind when reserving your classes initially! Space is limited and we want to be sure that every spot available in class is being used.

I understand that monthly tuition will not change due to the actual amount of weeks in the month or due to holidays or absences. Make-up classes are permitted within two weeks of an absence.

Summer Drop Ins are non-transferrable and non-refundable. Please keep this in mind when booking class.
