You must be over 18 to register for a Portal Account.

In accepting this Release, I acknowledge and represent that I have fully informed myself of the content of the waiver and hold harmless agreement by reading it before I sign it, and I understand that I sign this document as my own free act and deed; no oral representations, statements, or inducements, apart from the written statement, have been made. I further state that I am at least eighteen (18) years of age and am fully competent to sign this agreement; and that I execute this release for full, adequate, and complete consideration fully intending to be bound by the same. I further state that there are no health-related reasons or problems which preclude or restrict my or my child's participation in this activity, and that I will pay any medical costs that may be attendant as a result of injury to me or my child.

Semester enrollment:
- Registration, recital, and costume fees are non-refundable
- A partial refund will be given for dancers who drop during the first 4 weeks of class. After the first 4 weeks of class, there is no refund.
- OADC does not prorate for missed classes.
-Notice of a drop must be given in writing. A verbal drop to a teacher, or friend is not acceptable. Failure to notify will result in forfeiture of tuition refund.

Company enrollment:
- Enrollment for the Company season (May to April) requires a non-refundable deposit in May and enrollment in both the Fall and Spring Semesters.Payment schedule will be sent at the start of each season.

Open Air Dance Collective reserves the right to use photographs and videos taken during classes, workshops, performances, or other affiliated events for the purposes of instruction, advertising and promoting OADC and its programs. Students, or parents of students who are minors, who do not wish to comply with this policy must notify OADC in writing prior to participation in class.

I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release Open Air Dance Collective, its affiliate dance studios, event sponsors, employees and dance teachers from all liability for loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, or performances. I also agree to abide by any rules, regulations and policies set forth by Open Air Dance Collective.

Semester payments are due on the first day of class. Any payments received after the 3rd week of the semester will incur a $25.00 late fee. No student will be allowed to participate in classes or company after the 5th week of classes if tuition has not been received. Returned checks will incur a $25.00 fee.

There is a $35/dancer, or $60/family, annual registration fee due upon registration for the program, or the first day of class for students who register after the start of the session. All accounts must be paid in full in order for students to pick up their costumes and participate in the year-end recital (including tuition, costume fees, etc.).

I recognize and understand the risks of physical injury inherent to dance and dance training and I fully assume those risks including, but not limited to, in-studio classes, online live classes, and online pre-recorded classes. I hereby release Open Air Dance Collective, its affiliate dance studios, event sponsors, employees and dance teachers from all liability for injuries sustained or illnesses contracted while attending or participating in any dance classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and hold harmless Open Air Dance Collective, its affiliate dance studios, event sponsors, employees and dance teachers for liabilities, costs and judgments arising from acts of omissions committed by me or my child which result in injury or damage to any person or property.
