WAIVER & RELEASE

I hereby, for myself or my minor child, release, waive, and/or forever discharge any and all rights, claims and causes of action for damages that may be suffered by me and/or my minor child as a result of any activities or preparation for activities at Susquehanna Dance Center and its officers, agents, servants, employees and independent contractors.

I recognize and voluntarily accept all risks associated with my and/or my minor child’s participation in activities, no matter how remote or unlikely. I understand that the activity contains an element of physical risk. I recognize the inherent danger involved and take full responsibility for my and/or my minor child’s actions and physical condition.

I further take full responsibility for my or my minor child’s level of participation in this activity. I have, and/or my minor child has, no impairment, physical or mental, that should preclude participating in activities. I understand that I can remove myself or my child from participating in activities at any time should I choose to do so.

I recognize that it is my and/or my minor child’s personal responsibility to learn, prepare, understand, and obey the rules for participation in any activity of Susquehanna Dance Center.

I expressly agree that the foregoing Waiver and Release of all claims is intended to be as broad and inclusive as it is permitted by the laws of Pennsylvania and that if any portion thereof is held invalid I agree that the balance shall continue in full legal force and effect.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at SDC or participation in SDC programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless SDC, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

WARRANTY
I warrant that I am of legal age and have the authority to grant the rights under this agreement and agree to indemnify Susquehanna Dance Center from any claims.
