FALL CLASSES BEGIN October 10, 2022.

As material consideration for Toni Casson, Dana Kurzawinski, Toni's Dance Academy and its employees teaching exercise, dance or any other activity to myself or my child, I warrant to you that I am or my child is in good overall physical health or has been released by a doctor to be able to perform physical activity. I understand that your activities may be physically strenuous, and I understand that you are relying on this warranty. I hereby waive any claim which may arise from injury or ailment resulting by reason of me or my child engaging in activities under your care or resulting from your instruction. I specifically assume all risk of injury or death and I hereby waive any and all claims against you or your employees for any injury or damage to me or my child by reason of engaging in activities under your instruction. I do hold you harmless from any and all liability, loss, claim, damage, expense, cause of action, suit, claim or judgment arising from such injury or death resulting from exercise, dance or other activity instructions.

I have read the Toni's Dance Academy classroom etiquette and rules page and understand it in its entirety. I understand that only students paying for instruction are permitted in the dance rooms and that no outdoor street shoes may be worn on the dance floor. I also understand the dress code as well as proper hair that must be worn for all classes. I agree to abide by all general rules set forth in the Toni's Dance Academy brochure.

I agree to abide by the payment policy which is that all tuition is due on the 1st of the month. I agree to pay any and all subsequent late charges if my month tuition is not paid by the 1st . I also understand there are no credits or deductions for missed lessons from my monthly tuition and will not deduct for such missed lessons. I also understand that there are absolutely no refunds should my child choose to withdraw from Toni's Dance Academy at any time.
