You must be over 18 to register a dancer. You must be the parent or legal guardian to register a minor under 18. I understand that I am responsible for payments associated with this participant(s). I understand that I will be given a handbook, and that I will read and abide by these rules. If there is a problem, I agree to bring it to the attention of staff at Elevation Dance Company.

I hereby waive and release, indemnify, hold harmless The Elevation Dance Company LLC and it's owners, teachers, and employees, directors, choreographers, from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages, and liabilities of every kind and nature, whether known or unknown, in law or equality, and I ever had or may have, arising from or in any way related to my participation in any of the classes, events, or activities conducted by, on the premises of, or for the benefit of The Elevation Dance Company LLC. By this waiver I assume any risk, and take full responsibility and waive any claims of personal injury or damage to personal property associated with The Elevation Dance Company LLC including, but not limited to using the facility and it's equipment in any manner, form, or fashion, and practicing or participating in classes, events, or other related activities on or off the premises.

I understand that by registering, I give Elevation Dance Company LLC the right to use my child's likeness for social media/advertising purposes. If this is an issue, I also understand that I need to individually inform Chris and Kahli.
