Liability Waiver and Acknowledgement of Risk
Balleraena Dance Studio will make all reasonable efforts to help our students avoid injury to themselves and to others.
However, it is important that Participants and their parents understand, acknowledge and accept that participating in any dance class, workshop, rehearsal or performance, or any activity at Balleraena Dance Studio, there is a possibility of physical injury.
By registering for any class, workshop, rehearsal or performance, the Participant or their parent hereby acknowledge and agree to be bound by all the terms set forth herein in this Liability Waiver and Acknowledgement of Risk of injury. Participant and/ or their parents voluntarily agree to assume all risks and responsibility for any injury or accident, which might occur to Participant or my child during any classes, rehearsals, performances, or activities at Balleraena Dance Studio.
Participant further agrees to release and indemnify Balleraena Dance Studio, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to Participant, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Balleraena Dance Studio.
I further voluntarily agree to waive my rights and that of my heirs and assigns to hold Balleraena Dance Studio, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. Participant understands that Participant or my children should be aware of my or their child's physical limitations and agree not to exceed them.
If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights. I further hereby voluntarily agree that, for no additional consideration, Balleraena Dance Studio shall have the right to use photographs of Participant (or my child) for publicity purposes.
My signature is proof of my intention to execute a complete and unconditional waiver all liability, either on behalf of myself, as Participant, or on my own behalf, to the fullest extent allowed by law.
Further, should any damages be awarded to Participant or to the parent or guardian, the damages shall be limited to the fees that Participant or parent has paid for participation in any classes, rehearsals, performances, or activities at Balleraena Dance Studio.
By e-signing my signature, I certify that I give Balleraena Dance Studio permission to use any pictures taken of my child in dance class on Balleraena's social media platforms (Facebook, Instagram, etc.). If this is NOT ok, please contact Miss Raena (balleraenadance@gmail.com) prior to signing this form and registering for any class.
