Release & Waiver Dancing and tumbling are strenuous activities from which serious injuries could arise. Carolina Dance District will not be liable for personal injuries, or loss of, or damage of personal property. Please inform Carolina Dance District in writing of any physical limitations your child may have. If you are in doubt as to your child’s physical abilities, please consult your physician before allowing your child to participate. I, the undersigned, represent that my child,, is physically able to participate in all activities of the program. In the event of an accident or illness, I hereby authorize the staff of the Carolina Dance District to take the necessary steps involving emergency medical treatment. I authorize the hospital and staff to treat my child for any illness or injury in my absence. I understand that I am solely responsible for costs incurred for any and all medical treatment. I also give Carolina Dance District permission to use photographs/images of my child for publicity purposes.
