(My family and) I agree to comply with all State Street Ballet Academy rules and regulations.
(My family and) I will notify the Directors of student absences and other issues in advance or as quickly as possible.
(My family and) I will act in a kind, non-harassing, non-discriminatory manner with all students, staff, patrons, volunteers, and other families: this includes any attempt to disparage or defame the goodwill and reputation of State Street Ballet Academy and its teaching artists.

REGISTRATION: Registration forms must be accompanied by a tuition and annual registration fee paid at the start of enrollment.
Please submit a complete registration form for each student. The registration fee is non-refundable and guarantees permanent class placement.
Once a student is enrolled for the session(s), no deductions or refunds will be made for absence or withdrawal without proper written notice. Class size is limited. Class may close when filled or canceled due to lack of enrollment.

MEDIA RELEASE: I give permission for photographs or video footage which includes my child to be used for the school's promotional purposes on television, newspapers, magazines, or any other media publications. Contact the Directors if you do not want your child included in promotional photographs or video footage.

CASTING/CLASS PLACEMENT: Casting for our performances and class placement are at the sole discretion of the Directors. Roles may be repeated for more than one year. We believe that any performance experience is valuable, regardless of the role.

VOLUNTEER REQUIREMENT: One parent per student is required to volunteer during The Nutcracker/Rudolph shows and during the Spring Recital. Alternately parents may choose to pay a $35 fee to opt out of that commitment for each show, which will be used to hire backstage help for the safety of our students.

LIABILITY/MEDICAL RELEASE: On behalf of my child I agree that State Street Ballet Academy (SSBA) and State Street Ballet staff/faculty shall not be liable in any way for any injuries sustained, illness or loss of property during attendance in our programs. Permission is granted for immediate medical treatment and transport for my child in case of an emergency.

COVID WAIVER: I acknowledge the contagious nature of the Coronavirus/COVID-19 and that State Street Ballet Academy has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other SSBA clients.
I voluntarily seek services provided by SSBA and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while interacting with State Street Ballet Academy staff, faculty, students, and families.
I hereby release and agree to hold Euphoria Studios LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may arise in any way in connection with any services received from SSBA. This liability waiver and release extends to the organization altogether with all owners, faculty, and employees.
