Risk
By consenting to this waiver at Swarm Studios, I understand and acknowledge that participation in dance/vocal classes may result in personal injury to bones, joints, muscles, tendons, internal organs, and other aspects of skeletal system and potential impairment of other aspects of the body, and in the rare occurrence, death, complete or partial paralysis or brain damage and property damage or loss. The definition of participating in a dance program includes but is not limited to: it is an activity that uses exercises and dance routines made up of elements from dance, stretching, gymnastics and yoga as a tool to strengthen the body, improve alignment, flexibility, body awareness, performance quality, expression and more for performances and occasionally competition. I fully understand these risks and agree to participate in a dance program voluntarily and at my own risk.
Media Release
I give consent for Swarm Studios to have the right to use, without payment or charge, any written information, photograph, video tape or other visual media of myself or my child taken during class, rehearsal or performance event for the purpose of furthering Swarm Studio objectives.
Studio Rules
We at Swarm Studios value time and hard work. Through the power of music and dance we cultivate self worth and respect for all people. Please be present, and on time. Leave all negative attitudes at the door and do not bring them into the studio where they can hinder your training, or the training of others around you. All students and parents are required to have a attitude of positivity and kindness and not discuss anything in regards to the studio in a negative manner, especially concerning another student, parent, or Swarm Studios personnel. Do unto others, as you would have them do unto you.
Financial Agreement
All students and parents are solely responsible for all costs incurred as a member of Swarm Studios including, but not limited to, administration/membership fees, monthly tuition, costumes fees or purchases, competition and travel expenses if applicable, regardless of circumstances that may arise. All fees are to be paid by the due date on the invoice. It is the responsibility of the student to attend all classes – fees will not be credited or refunded if you are sick, injured, choose to remove yourself or have other commitments. Any fees not paid by the start of week 3 will incur a late fee of $10. We reserve the right to set up an account with a debt collection agency. We will give you appropriate notice if this takes place.
Cancellation Policy
We require 2 weeks' written notice before the term ends if you or your child wish to unregister. Failure to do so will result in families incurring a 50% cancellation fee of the following terms tuition fees. Due to long waitlists, it's unfair on students waiting and the studio to miss out on business.
Intellectual Property
Any choreography that is given to the students is Swarm Studio’s intellectual property. Using or sharing, on any platform, the choreography must be discussed with the Tutors and Studio Director.
