By accepting this waiver you have received the following information:
A paper contract must be signed on the first day of class in order to participate. The verbiage of that contract is included below.
Tuition and Charges
Enrollment fee: An annual fee of $30.00 is charged for the first dancer and $15.00 for additional dancers in the same household.
Tuition: Is divided into 10 equal payments (Sep-Jun). Tuition is the same each month regardless of attendance, school closures, holidays, vacations, etc. Payment is due on the first of each month and considered late if not paid by the 10th of each month. We accept payment in the form of cash, check, debit/credit. Discounts are given for multiple school year classes.
Tuition for the 2023-2024 school year will be as follows:
30 min classes $32.50/month
45 min classes $48.75/month
60 min classes $65/month
75 min classes $81.25/month
Accounts with tuition payments past due more than 30 days may result in the dancer not being allowed to participate in class until the account is brought current.
Pay in full discount applies to registration received prior to the beginning of the school year session. Early withdraws will result in charges reverting to full price.
NSF fee: A fee of $45.00 will be applied to your account for all returned checks.
Late Fee: A $10.00 late fee will be applied to your account if tuition is not received by the 10th of the month (including holidays and weekends).
Collections: Accounts that are past due by 90 days are subject to collection.
Costumes: A costume fee of $70.00 per class is required to perform in recital and must be paid by Winter Break 2023. If paid after that date, the cost will increase to $85.00 and costumes aren’t guaranteed to arrive for recital. Tights and shoes are not included with your costume. Costumes are non-refundable and cannot be canceled. Costume fees paid for and then refunded before orders are placed will be refunded minus a $10 costume deposit. Measurements for each dancer are taken in class and allowances are made for sizing up due to possible growth of the dancer. Costumes are ordered using the sizing chart from the specific company that costumes will be ordered from. Slight alterations may be required (hems, straps, etc.) and are the responsibility of the dancer/parents.Costumes are tried on in class prior to the dancers taking them home to ensure fit. Major issues should be brought to the attention of Emily.
Dancers that drop class after costumes are ordered and paid for will be notified when their costume arrives. The costume must be picked up and signed for by an adult prior to the recital date or it will become the property of MPAC.
Recital Fee: Participation is encouraged but not mandatory. If you do not plan on participating, please notify your instructors in writing no later than February 1st. Classes will end the last week of May for those not participating in recital. To cover fees incurred for recital, a $70.00 fee per family will be added to your account on March 1 and is due no later than March 31, 2024. Those choosing to participate in recital will be required to perform in both shows.
Accounts: All accounts must be paid in full prior to rehearsal. Accounts in arrears will result in the dancer not performing in recital.
Re-enrollment fee: If a student drops class for any reason and decides to return at a later date during the school year, a fee of $30.00 will be charged for re-enrollment.
Recital Tickets: Tickets will be sold prior to the recital at a cost of $12.00 per person. All adults and children 3+ attending recital are required to have a ticket.
Policies
All precautions will be taken to prevent accidents. Simple first aid will be administered to minor injuries. Paramedics and/or ambulance may be called when necessary and payment for these services will be your responsibility. It is hereby agreed that I, and my heirs and executors waive and release all rights and claims for damages that I may have against MPAC Dance. The risks involved in respect to such program are fully understood. This release is valid at all terms or months my family is enrolled or participating in programs/events.
I also understand MPAC Dance has photographic rights and photo and/or films may be taken and used for publicity purposes. Dancers performing in recitals/performances will be in MPAC’s production DVD. Music copyrights are paid by MPAC annually and it is illegal to copy any DVD’s.
Attendance: Regular attendance is important and ensures progress for all dancers and the completion of choreography in a timely manner. A dancer that misses more than three classes after February 1st may forfeit dancing in some of the recital routine or be subject to the cost of a make up lesson(s). If your child will be absent, please contact the studio.
Arrival/Dismissal: Dancers should arrive in dance attire 10 minutes prior to the class start time. Classes may be adjusted within 5 minutes of their scheduled times.
Inclement Weather: We will update the phone by 3:00 pm with closure Information. Please call 360-651-9000. Classes missed due to inclement weather may be made up by attending a comparable class (if space allows) within one week.
Teacher Absent: If a teacher is absent, there will still be class as usual. There may be a substitute teacher or classes will be combined.
Placement/Class size: MPAC reserves the right to cancel, combine or divide classes to ensure proper class size. Class placement is determined by the instructors and based on dancer’s ability,
Drop: If a dancer chooses to NOT attend classes, we must be notified and a DROP form must be completed and signed. Your account will continue to be charged the monthly tuition fee until that DROP form is filed with our office. If a dancer drops a class after February 1st, a $50.00 re-choreography fee for each class will be applied to your account. Four consecutive unexcused absences will result in MPAC dropping your student from class and fees up to that point will still be due and payable.
Rehearsal Date/Recital Date/Class PictureDate/Closure Dates: As soon as these dates are determined they will be posted in the lobby for your convenience.
Lost and Found: MPAC is not responsible for lost and/or stolen items left in the dressing rooms, dance studios or lobby.
Private Lessons: Private lessons are available and must be scheduled with the instructor.
Payment is due at the time of scheduling. Cancellation must be at least 24 hours in advance or you will still be charged the full payment amount.
Classroom Attire
Dress Code: Attire must be appropriate for each form of dance.
Ballet-Leotard and tights, ballet shoes.
Jazz-Leotard or tight fitting shirt, jazz pants or leggings and jazz shoes.
Tap-Leotard or tight fitting shirt, jazz pants or leggings and tap shoes.
Hip Hop-tight fitting shirt, leggings and tennis shoes (for studio use only). No jeans or baggy pants.
Modern/Lyrical-Leotard or tight fitting shirt, jazz pants or leggings and bare feet.
Hair -must be in a bun or snood for ballet classes and a ponytail for all other classes.
Gender Neutral/Boys-Black leggings/pants and a plain white shirt are appropriate alternative attire.
Classroom Etiquette
Dancers should arrive to class on time in proper dance attire.
It is advised that students bring a water bottle (plastic) to class.
Dancers are asked to use the restroom before class begins to avoid classroom interruptions.
Dancers are asked to not enter class until invited into class by the instructor.
No phones or cameras allowed in classroom.
Parents are asked NOT to open the classroom door or enter studio during class. The instructor will come get you if you are needed.
Corrections given in class are a necessary tool to help your dancer progress.
Mandated Reporting
All staff employed with MPAC are mandated reporters. MPAC will protect children from all forms of child abuse or neglect. MPAC has a duty to report and are required by mandatory reporting laws to report any suspected physical, sexual or emotional child abuse, any suspected child neglect, child endangerment, or child exploitation, a child's disclosure of sexual or physical abuse and maltreatment to Child Protective Services (CPS) (without prior notification to the parents involved). All staff or volunteers in this program, as well as family members, are trained on prevention and reporting of child abuse, neglect, sexual abuse, maltreatment or exploitation.
Best Fit
Dancers and dance families should maintain a courteous demeanor towards our staff, other dancers, other families and any representatives of MPAC such as volunteers and/or chaperones.
Vulgar language, swearing and rudeness will not be tolerated at our studio or any outside venues in which we perform or participate.
Proper theater etiquette will be required. Guests are asked not to enter or leave during a performance. Please wait to do so in between numbers, as not to disturb other guests in the theater.
Failure to follow guidelines and policies or staff requests may result in future dismal from MPAC. Rudeness or profanity used towards staff will result in immediate dismissal.
