I hereby consent to and authorize the use and reproduction by Jax Ballet, LLC and all company affiliates any and all photographs, recordings, videotapes and/or other reproductions of my child's likeness for any purpose, whatsoever, without compensation. All images shall constitute the property of Jax Ballet, LLC and all company affiliates solely and completely. Further, I assign and release all rights to said images and authorize Jax Ballet, LLC and all company affiliates, or others authorized by them, to exhibit, broadcast, and/or distribute or otherwise further reproduce said images in whole or in part over or in any medium whatsoever, including newsletters, radio, newspapers, film, cable and television.

I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Jax Ballet LLC/Jax City Ballet, its owner, faculty, instructors, and all affiliated entities. I hereby agree to release Jax Ballet LLC /Jax City Ballet and hold Jax Ballet LLC/Jax City Ballet harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation. If I am a minor, my parent and/or legal guardian has signed this document releasing Jax Ballet LLC /Jax City Ballet from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity. The participant has my permission to participate in Jax City Ballet events. I further release Jax Ballet LLC /Jax City Ballet of all liabilities associated with my childs attendance at Jax Ballet LLC/Jax City Ballet.

Tuition and Payments 2024-2025 Season

The annual registration fee of $60 per dancer and $85 per family of 2 or more is due at the time of registration.

Monthly tuition is paid to keep a student's spot in the class. It is the same each month and is not dependent on how many classes are attended that month. Tuition is based on an annual average of 4 weeks per month. Tuition does not change because of holidays, studio closures, or recital photo days, rehearsals, or performances. So tuition is the same no matter how many weeks of classes there are in the month.

Registration is required 24 hours prior to class.

Our tuition prices are already accounting for holidays and breaks.

Payment is due on or before the 1st of each month.

If your account has any outstanding balances, the student will not be allowed to participate in school productions or events until paid.

Tuition refunds are not given if you miss a class or drop part way through a month.

If a students account is delinquent for 30 days, they will not be allowed to take class until account is current.

If you begin class in the middle of the session/month, tuition amount due will be prorated.

All registration and tuition fees are non-refundable and non-transferable.

Auto-Deduct:
Enrollment in Auto-Pay in the Parent Portal is required for Semester/Season registration.
All tuition is billed by auto-draft on the 1st of each month. Failed charges have 7 days to settle the bill and get your account updated with correct billing information, or a $30 late fee will be applied to your account.
