I hereby agree to release Jamie Campbell's Dance Company, LLC harmless of all liability, and hereby acknowledge that the student and I knowingly and voluntarily assume and accept full responsibility for all risks of injury or damage resulting from such dance activities arising out of active participation in on behalf of the participant. The student's participation in the activity is voluntary, and the student elects to participate, and I join that election, in spite of the risks. That in the event I choose to leave my child and/or ward before, during or after a class, workout or a performance, I hereby give Jamie Campbell's Dance Company, LLC my permission to use its discretion in determining whether my child and/or ward requires medical attention and, if so, to use its discretion in transporting my child and/or ward, selecting a healthcare facility and obtaining treatment for him/her. That in my absence Jamie Campbell's dance Company, LLC does NOT assume any responsibility for the care, custody, control, condition, health or well-being of my child and/or ward.

I understand that class placement will be evaluated by the director or designated staff of Jamie Campbell's Dance Company, LLC during the first few weeks of class. I understand that if the director/instructor feels that a student was incorrectly placed in a particular class that they will be moved to the appropriate class.

Due to unforeseen circumstances and in the event that a student needs to withdraw from class, Jamie Campbell's Dance Company, LLC requires a 14 day written notice prior to their anticipated last class. If written notice is not received, Jamie Campbell's Dance Company, LLC will continue to bill until notice is received. All tuition, costume and other associated fees are non-refundable and must be paid in full by their anticipated last class. Jamie Campbell's Dance Company, LLC reserves the right to withdraw a student from class/studio in the event that they exhibit unsafe behavior, toward themselves, our students, instructors, staff and/or other family members. The use of foul language or negative behavior will not be tolerated, by parents/guardians or students.

The undersigned gives permission to Jamie Campbell's Dance Company, LLC, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

In order to provide a safe environment for our students and families, we ask that you kindly adhere to the following: Jamie Campbell's Dance Company is located in the lower level of the Highland Medical Building. All students should arrive at the dance studio through the lower level entrance at the rear of the Building. Please bear right when entering the parking lot to drive around Building to the rear entrance. If you are dropping off or picking up your child, please utilize the designated "drop off/pick up" parking spots. If you are planning to remain on the premises while your child is attending class, please park in any of the non-designated spots at the rear of the Building whenever possible. Tuition is due by the first Friday of every month. I understand I am subject to a $15.00 late fee if accounts are not paid after 10 days past due, as well as a $35 returned check fee, should a check bounce. I agree that if the students' payment is 15 days or more overdue in tuition or other fees due, the student will not be allowed to participate in the dance curriculum, but may observe the class until payment is made with permission of the director at Jamie Campbell's Dance company, LLC. I also understand that I am responsible for all tuition payments as described in this tuition schedule. I understand that tuition or any other fees paid to Jamie Campbell's Dance Company, LLC is neither transferable nor refundable, including registration and performance fees. I understand fees and pricing are subject to change. Tuition payments will remain the same regardless of holidays, school vacations, and inclement weather.

I grant Jamie Campbell's Dance Company, LLC permission to use my child's photograph/video, with or without their name, in or on any form of marketing, publicity, illustration, advertising, social media, marketing, and Web content for Jamie Campbell's Dance Company, LLC or an affiliated event.

As the legal parent or guardian, I release and hold harmless Jamie Campbell's Dance Company, LLC, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Jamie Campbell's Dance Company, LLC, its owners and operators or in route to or from any of said premises.

I agree that prior notification of billing will be provided by Jamie Campbell's Dance Company, LLC in a timely manner via email. I authorize Jamie Campbell's Dance Company, LLC to charge the debit/credit card provided, according to the terms outlined. I understand that this authorization will remain in effect until I cancel it in writing, and I agree to notify the business in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. I certify that I am an authorized user of this credit card or bank account and that I will not dispute the scheduled payments with my credit card company provided the transactions correspond to the terms indicated.
