Waiver of Liability
I, as the parent or legal guardian of the registered student at Artists in Motion Performing Arts, understand that there are risks of physical injury associated with, arising out of, and inherent to the activity of dance. In recognition of this, I agree to release Artists in Motion Performing Arts and hold them harmless of all liability and hereby acknowledge that I am knowingly and voluntarily assuming full responsibility of all risks of physical injury arising out of active participation in a dance or tumbling class or other arts related activities with regard to your student.

Photo Release
I give full rights to Artists in Motion Performing Arts and its staff to use photos and video images of me or my child to use for promotional purposes of Artists in Motion Performing Arts only. Photos and video will be used in brochures, websites, advertisements, and other promotional material created by the studio. Photos may appear with or without names in press releases and other advertising.
